I love the story of an African president who tried to do something about the AIDS problem by distributing tens of thousands of condoms for free, as well as an information pamphlet that explained the dangers of unprotected sex and how to use a condom. The problem was they stapled the pamphlet to the condoms, poking holes in all of them, rendering them all useless. Then they had to go around and collect the damaged condoms saying they didn't work, shattering any trust the people had in condoms.

Happened to me once. Living and working in rural Southern Africa.
