  • Doctor Andrea Furlan explains that osteoarthritis is a slow and low-grade inflammation that attacks the cartilage of the joints. The amount of pain a person has is not a good measure of how much damage there is in the cartilage. Some people have a lot of damage and not much pain, and others, have mild damage and feel a lot of pain.
Dr. Furlan explains what the drivers of ongoing chronic inflammation are, and how to prevent its progression.
Osteoarthritis affects a lot of people who are overweight and obese. The adipose tissue, or fat tissue, produces substances called "adipokines" and these substances maintain the pro-inflammatory state in our bodies. This pro-inflammatory state leads to the ongoing destruction of the cartilage that protects the joints.
If you want to slow the progression of osteoarthritis and decrease the intensity of pain, try these strategies: 1) lose weight, 2) move the joints as much as you can, 3) avoid central sensitization doing mind-body therapies, and 4) start an anti-inflammatory diet.

I was diagnosed with osteoarthritis in my early 60’s, but probably the diseasewas there already many years earlier. My fulltime job was gardener, on hands and knees a lot of the time and always on the move. I have never been obese or even overweight. Wear and Tear was the likely cause of my hip pain. Now I am almost 70, retired for two years and the pain comes and goes in the course of a year. I hardly ever take ibuprofen but if I do it works. I do tai chi, other exercises, walk and undergo radon therapy once a year. This therapy works a dream. I try and avoid any operation and artificial hip replacement. So far so good. Thank you for your informative video’s. They are very helpful.

Thank you for this! I was told that it’s not from inflammation, but from wear and tear. I knew that wasn’t the case because, firstly, I’ve been careful with my joints my whole life. But, secondly, when I’m eating non inflammatory foods, my pain is close to zero. Literally an hour after eating sugar, my joints go crazy for a few days.
Also, movement does really help me.

You could research Grounding, also called Earthing. The expert on this is Clint Ober and he has many videos and a website. I’ve been carefully studying his information and think it has validity. It can’t hurt, and it’s worth a try. It’s too cold out right now, but I will definitely be trying it for my hip and back pain. I will also buy a grounding pad for the house. Unfortunately, they don’t ship their products to Canada, but we have some items up here. Just a thought. Actually, when we had a couple of milder days, I tried it outside and I think it helped, temporarily, but maybe my imagination. I try everything, if it’s not harmful. Any improvement is welcome.

You are my go to doc for chronic pain. Great vid.
For arthritis, I like these tips too:
These are really great tips, doc. Here are also some tips your viewers could use:
    1. Regular Exercise:
Engage in low-impact exercises like swimming, walking, or cycling to maintain joint flexibility and strengthen muscles. Consult with your healthcare provider or a physical therapist to create a suitable exercise plan.
    2. Balanced Diet:
Consume a well-balanced diet rich in anti-inflammatory foods such as fruits, vegetables, whole grains, and fish. Omega-3 fatty acids found in fish may help reduce inflammation.
    3. Medication Adherence:
Take prescribed medications as directed by your healthcare provider. Consistency is key in managing symptoms and preventing flare-ups.
    4. Joint Protection:
Use assistive devices or adaptive tools to ease strain on your joints. For example, ergonomic tools in the kitchen or using a cane for added support while walking.
    5. Hot and Cold Therapy:
Apply heat or cold packs to affected joints to alleviate pain and reduce inflammation. Heat can help relax muscles, while cold can numb pain and reduce swelling.
    6. Manage Stress:
Practice stress-reducing techniques such as deep breathing, meditation, or yoga. Stress can contribute to inflammation and exacerbate arthritis symptoms.
    7. Adequate Rest:
Ensure you get enough restorative sleep. Establish a regular sleep schedule and create a comfortable sleep environment.
    8. Weight Management:
Maintain a healthy weight to reduce stress on your joints. Losing excess weight can alleviate pain and improve overall joint function.
    9. Regular Check-ups:
Keep regular appointments with your healthcare provider to monitor your condition, adjust medications if necessary, and address any concerns.
    10, Stay Informed:
Educate yourself about rheumatoid arthritis to better understand your condition. Being informed empowers you to make lifestyle choices that positively impact your health.
Remember, it's crucial to work closely with your healthcare team to tailor a management plan that suits your specific needs and to address any concerns or changes in your condition promptly.

Ok so that would mean people who are thin they don’t have osteoarthritis? That makes no sense. Plenty of people like myself who have always been thin, exercise regularly, eat a non-inflammatory diet still have osteoarthritis

Hi Miss Bardot! There are many other risk factors for osteoarthritis. Mediicne is not an exact science like algebra.

I've learnt somethings & I've had a very important point reinforced, that is "Use it or Lose it!" When I'm feeling stiff all over my body I do stretches, then hop on my Treadmill & then Rowing machine. Then I workout on my Bullworker for strength, and after an hour, I feel like a man half my 70yr old age! So much more flexible. The fact that there won't be any damage done to the joints during the exercise phase, is so reassuring. Thank you Doctor👍 p.s: 9yrs ago, my Orthopaedic Surgeon highly recommended I have an immediate double knee & double hip
joint replacement, but I declined & had my left knee meniscus removed only! I live a full healthy life, relatively Pain Free.

I have a displastic left hip since I was born. Basically my joint socket is too shallow to cover the whole joint, therefore lot of friction happening there ,causing this inflammation, so clearly my osteoarthritis in my left hip was caused by the wrong mechanics and wear and tier, since my other joints are ok.

Dra.

I don't know what I'd do without Turmeric as I have severe osteoarthritis throughout my body. Interestingly, it started when I was in my 20's and a good weight. 50 years later it has progressed but I move a lot and take Turmeric every day and generally only feel pain when I get up in the morning. I'm still trying to eat a healthier diet but it seems to be a lost cause as my body get really cranky when it isn't fed the carbs it wants.

I'm baffled. I injured my left shoulder in the gym 6 years ago. I had progressive pain for all those years, until X ray, then CT scan and MRI showed that virtually all the cartilage was destroyed. I felt I had no option but to go ahead and have a shoulder replacement.
However, I have always been slim and active (apart from pregnancies!) and have no signs of osteoarthritis anywhere else in my body - and I'm saying this because all my other joints move freely, although a couple of fingers are becoming harder to bend.
Also had Dexascan last year which showed low risk of fracture.
I honestly don't understand what my body is playing at. How could I lose cartilage and be grinding bone on bone in one joint only? I must be missing something. So frightened it will happen to other joints. 😢

Thank you so much for this. My recent X-rays show mild arthritis in lower back, left hip and left knee but have just been through a flare up which made me wonder what it would be like if the changes were severe. Disrupted sleep, or no sleep. However it is improving, less pain freer movement but leaves me thinking, why is it better when the arthritis is obviously still there. I do need to lose some weight but interesting that it is not just the weight on the joints. I will be eating a more plant based diet and pushing myself with the exercise instead of guarding myself. I will also be watching all your videos. Feel more hopeful for the future.
