If you all haven't notice, there is the dialogue of Commander Saying Quote:
"From what we've gathered patient zero wasn't just the first subject...
But it also refers to the scientist responsible for the experiments!"
This could possibly mean that Patient Zero was actually the first one to be "infected" as a zombie. If you don't know already but the term "Patient Zero" is often refers to as the First Patient or subject.
And also my theory for the other Zombies existing in the trailer as Patient Zero walks out is probably because the fragment has made a large Radiation Wave after it was put in the possibly "radioactive compound"

Great theory as always. i myself went over and checked some of the more hidden details in the trailer itself, i have found out that apperantly they used some alternatives of compund 3 which was originally the cube fragment. but it only gave off side effects rather than empowering them completely, which is interesting to note for later. as for Patient Zero i do think the cube doesn't only grant the vessel just random powers, it gives them ones based on who they are, Umbra got more solar powers due to her having some sort of connection with it and also witch craft, however Patient Zero got more scientific powers in a way if you know what talking about, but it could be false as the cube was put in nuclear waste like you have mentioned, so now there is raw power and mixture power, the first one is obvious as Umbra herself just touched the cube, and the other one is the fragment being used with another substance or another material. one other thing to note is that the cube itself grants far more power than a fragment of the cube, as Krampus itself had the same sort of attacks and health as last time due to him only having a fragment. but Umbra got complete power over armies from legacy bosses like Jaxe and Swamp Monster being fron Nightmare Carnival or well Jack O' Bot, and not to mention powers such as being able to damage the base of the player, and another thing is rage mode as every boss that had the fragment has a rage mode but it could be a coincidence. whatever the next Era of TDS we surely are going to meet those who were hungry for power and got their hands on the fragments of the cube.

theory about the solar eclipse event (absolutely not the theme): Basically at the end, umbra glitches.
The theory is that the creator has made a some sort of dimension, something like hell that sucks in powerful creatures.
Because umbra was the strongest at that time.
Also narrator from 2020 is the same as 2023, as well as the creator of Jack O Bot
I feel like patient zero is like a royal scientist to the fallen king, like alphys or gaster to king asgore. He was assigned to make a stronger army for the fallen king using fragments of the god cube found over the fallen kings land, but as more failed experiments would happen patient zero tries to make one that works but since no one would want to participate to the failure of the experiments he would try it on himself and lead his experiments himself and not for the fallen king.
Patient Zero could actually be the cause of PW2 and Badlands game modes + The one that created all of the normal enemies (Since they actually appear on Normal mode [Strongs] ) and the hazard enemies could explain how PW2 is related to him, and i think why is that the liquid that Patient zero created is the same one used in PW2 due to the similar appearences of both game modes.
Now i wonder, who the fallen enemies are.. Maybe it is related to the kingdom from halloween event 2023?
He used the purple shard from the power cube which makes him very strong

Theory: the shard fragment of god cube only affects patient zero, most likely the glass broke after the shard was mixed and then exploded with zero in the radius of the explosion. thus, creating patient zero.
Another theory: the badge for beating his mode says "Destroy the apocalypse from its roots!" which probably implies that he created grave digger too, not too sure for molten boss since he's volcanic energy, but zero probably used a potion to create him, like dropping a potion in a tower of lava (as tall as the towers in polluted wastelands). I assume he created molten boss cause of the slimes capable of shape shifting which zero probably used this trait to make lava shapes shift
That second theory about molten boss reminds me of the calamity lore, love it.
what was his motives to destroy humanity if u turn on subtitles he says some words in english but he says in german
"Now it's my turn to wipe out humanity" Which is weird cuz he also says they are already wiped? what does he mean its such a riddle i need help also if the lunar overature event was supposed to be a reenactement of the events that happend then us not dying that he says " we arent following the script by surviving" means that in the events of the game we are supposed to die which means that the cube was destroyed at that point and possibly lead to the curroption of umbra since she was linked with its power also this same one seems to be related somehow to the hardcore boss he looks like the cube somewhat u know? also in the note it seems that his name is zero so perhaps things got a bit mixed so commander thought he was patient "zero" the number when its just the name of this patient u know?
Tbh zero is kinda disturbing, in the trailer for inter we see him working in some hospital or asylum and here is the creepy part of my theory. He went into a hospital/asylum to test the cube fragment and that means he could have tested it in patients probably even the workers of that place, and considering what they look like my guy probably lobotomies them and made them feel a lot of pain. Hope I probably made you question tds:)

So. The enemies of intermediate mode is pretty similar to pw2. Amd back in the days. There was a healing enemy in pw1 (I forgot his name) so I have a theory. What if polluted wasteland is a factory that has a waste of the cube? And also what if it's ran by patient zero?
Like I said, we cant take everything for granted. In the banner it says the facility's name, meaning there could be more operations. If we also look at the scientist's paper, it said that it needed compound 3, being the god cube, which means they didn't have it to begin with.
Well, now im thinking why his name is "Patient Zero", probably because that's just his name.
Well I have a theory of that, maybe the scientist was a prisoner once aswell but he got to become a scientist one day, his name still being "Patient Zero", but it could be just his name to sound cool.

he was also partially corrupted by the cube before the explosion and he is literally that one crazy scientist. i also think that god cube fragment which caused the explosion was capable of keeping zero's mind pretty much as it was, he is still intelligent despite being infected. all entities affected by the cube fragments seem to keep their memories etc, theyre just turning evil, zero, umbra, penumbras etc
i wonder if thats a scheme which will be followed in the future gamemode reworks
“Patient Zero” is a term used to refer to the first possessor of an infectious disease. Probably refers to the fact that he is the first individual to possess / be corrupted by the powers of the God cube (at least for a good while since Umbra). This also means he’s likely the progenitor of the whole zombie apocalypse that we’re fighting in the main gamemodes.
Heck, he might’ve even intentionally created all of the other bosses of the other modes by using the God cube solution he used on himself on some dead subjects (Grave Digger, Fallen King, Gunslinger, maybe even Wox the Fox) and even inanimate matter (Molten Boss, Nuclear Monster). Not quite sure about Void Reaver though, I suspect he’s just doing his own thing separate from Zero.
Either way it’s all very interesting and I can’t wait to see the reworks of the other modes add to the lore.
