Follicular Unit Transplant (FUT)
Follicular Unit Transplantation (FUT) is the primitive technique of hair transplant. It is popularly called ‘strip method’ where a strip of scalp skin containing hair roots is taken from the back of the scalp. This strip is cut under microscopes into individual ‘grafts’ containing hair roots. These grafts are planted on the bald area. The area of the scalp from where the strip is taken is sutured and this wound heals in a week or two. Newer methods like Follicular Unit Extraction (FUE) have slowly replaced this technique.
Follicular Unit Extraction (FUE)
The method of extracting grafts is the only thing that marks a difference between Follicular Unit Extraction (FUE) and Follicular Unit Transplantation (FUT). In Follicular Unit Extraction (FUE), each hair graft is removed from the donor region one by one using fine punches of caliber less than 1mm and is stored for about 3-6 hours in a storage solution. After creating slits (space for putting hair grafts) these stored grafts are then implanted one by one into the recipient site. The site from where grafts are taken heals in 2-7 days without apparent scarring. This is also called ‘Stitch less hair transplantation’.
Direct Hair Transplant (DHT)
This is the most advanced technique of hair transplantation. It is a modification of Follicular Unit Extraction (FUE) technique. In this technique, first slits are created in the recipient bald area. Then grafts are harvested from the donor area and transplanted within minutes (2-20) to the recipient area. The chances of grafts survival is almost 100%. Since all the hair grafts survive, maximum density is achieved only with Direct Hair Transplant (DHT).

Hello doctor, if i done my FUE transplant and after some years if i have to undergo another surgery how will you plan my second session of transplant? Do you practice of extracting hairs by FUE for second session too? Further i have seen lot surgens or HT clinics these days prescribed 1mg finestride per day upto 4 or more months for quick results. As we all know this medicine can show a great results during its consumption which will ultimately go after stopping it. But my question is how will you manage the tolarance effect that our body will built against this medicine? I have found many new technologies for FUT which can give a smooth result without any scars and those part of donor area again can be used for transplantion. In this scenario why do you only focus on FUE telling that FUT is a old and ineffective method of HT? Many surgeons from india as well other wetern countries promtoing a combo and solo method of FUT and FUE, however why all Neplease surgeons are only talking about of FUE only. Dont forget that people are more aware than before nowdays and we know that transplanted hair can vanished too in future and our existing hair may not be restored 109 percent just with PRPs and finestride. So we might need our precious donor area for future use. Are any clinics in Nepal are concerned about these hidden truth about hair surgery??? Exepecting a genuine reply please.

Planning a second session with FUE depends entirely on donor area availability. After the first session of FUE we expect some degree of improvement in interfollicular distance i.e. distance between two follicular units because of contraction during healing. So many surgeons around the world are doing a second session of FUE depending upon the contraction achieved.
You are right, FUT is also another method which can be planned and many experienced surgeons leave a very fine scar line which is barely perceptible. I am not against any technology of hair transplant but my recommendation is based on complications which I observe after different techniques and I have observed a good number of patients with ugly scar, infection, persistent pain etc. after FUT technique. Having said this, I re-emphasize that I do not mean that FUT is inferior to other techniques but there are more chances of complications if not done in good hands.
Combination of FUT and FUE yields more grafts and this is always preferred in advanced grades of baldness where we need maximum grafts.
The most accepted theory of hair transplant till date is “DONOR DOMINANCE” and there is no scientific explanation for transplanted hair falling off. But a number of reasons are proposed for thinning of transplanted zone after hair transplant
1) If grafts are harvested from unsafe donor area, these grafts are susceptible to fall in future
2) If hair transplant is done in area with preexisting hair for density augmentation, there is a probability that the pre-existing hair might fall in the future leaving behind only transplanted hairs
3) Patient developing condition which leads to diffuse hair fall unlike pattern hair loss leading to fall of transplanted hairs. Some of the causes of diffuse hair loss are thyroid disorders, medications, malnutrition, heavy metal exposure, etc.
Regarding tolerance to medicines, this is something new which I have come across. I would request you to send me a reference of information which mentions tolerance to medical therapy in long term use.
