I would have set Up to move Forward, Down to move Backward, Left and Right just to turn. I know there's no strafing but you can move and look around, at least.
Start should be for the Pip Boy menu. Select should be to reload/draw.
B as Fire
A as Interact
New Vegas has a few distinct control issues, mainly to do with VATS. Without the ability to look up or down, you would have a very hard time in firefights without VATS. I theorize that whatever button registers your VATS input should be set to A, and also a button to cancel VATS should be set to select. B will, after all, be the button to fire with.
