Hi. Thanks for these kind of videos. I'm a retired nurse but had no experience with trauma so am trying to learn all I can. I guess the arm you were using threw me as the hand had an amputated thumb. Are you packing the wound because it doesn't have an arterial bleed and it's just a big hole? I was assuming while watching the video that you were packing because bleeding was a problem. When you said at the end that they should have a pulse distal to the wound, threw me. Thanks in advance for your help

Hey fellow RN here. My understanding is that the wound is tunneled and bleeding won’t stop. So packing to make direct contact is the best bet. Otherwise if you just cover without packing it will continue to bleed as there is no direct pressure on the internal surface area of the wound. Think GSW. The reason we ant a distal radial pulse after placing the bandage is due to us wanting to avoid the tourniquet effect and to prevent damage to the distal tissue in the hands etc. The Israeli style bandage placing just the right amount of pressure over the wound after packing to stop the bleed while maintaining a distal pulse.
