well if you keep your phone too close to your eyes and don't take any rest every 15 minutes your eye muscles will adopt to the view distance between your eyes and your phone and when you try to look in bigger distance it will be blurry 'cus your eye muscles have concentrated to smaller view distance

Drink Carrot or Beetroot juice twice a day, perform eye exercises and check if it helps. All the best ❤️
