Sai Pallavi was approached by Alphonse Puthren, a movie director, while he was studying medicine at the Tbilisi State Medical University. She was offered the role of 'Malar' in the 2015 Malayalam film, 'Premam' opposite Nivin Pauly. She finished filming the movie during her holidays. Fans often recognized her as Malar instead of Sai Pallavi because her role as Malar was so well received.
