I just checked about the OGTT , is not a simple blood test because it requires one to be several ours at the lab. The question is : can someone "mimic" it at home drinking the indicated glucose solutions at the right intervals and check blood with a home device?

Engineering Perspective: The CAC score is a picture of your historical endothelial damage and progression to plaque: all in the past. What happens in the future is in your control. If you remove inflammatory factors by all the things we know, the probability is that plaque progression will reduce (or even stop). And just as important, existing plaque will tend to stabilise

I feel comforted about my husband after this video! He has a high CAC score but has changed his ways to be healthier. His former doctor scared us to death, and made us worry he might not make it back to the parking lot!😮
I like Dr. Gil! He’s one of the most objective, unbiased people out there reporting on the data. He’ll be a great guest!
Left my prior comment before the program ended and just had to add this: Dr Brewer, your empathy and respect for people really comes through, signs of a caring person and a great doctor.
I attended the conference in St. Pete and can say it was the best, most valuable two days of information and learning; somethig not found anywhere. Discovering Dr. Brewer and the PrevMed channel has been a game changer for improving my health. I enjoyed sharing stories with other attendees and can't thank Dr. Brewer and everyone on the team enough for the work they did to make this the best two days of easy to understand, meaningful information to have better health. Also, Daniel Trevor's book 'Unholy Trinity' given to each attendee a must read! I have read it and have purchased 3 more to give family members to read. It could save a life. Can not thank all of you enough for your excellent work.
Average American daily calorie is 3600
Sugar went up by 100 calories
Vegetable oils went up by more than 600
Yea sugar is bad,
but being 100 pounds overweight cause 3600 calories
Totally enjoyed the scientific content, camaraderie and presentation of format...
What if we could easily test our own fasting insulin at home and also our glucose levels and then get a HOMA IR - would this not be a good idea?
So one thing I want to ask of you, please. All my tests showed I was a Rockstar on paper. Nuclear tests were unremarkable. Supposedly, a very low risk even with a 706 score based on bloodwork and multiple diagnostic tests. I DID go down the bunny hole you speak of often and pushed for more tests - call it a gut feeling. I'm glad I did. I agree with everything you say with one caveat. None of the tests showed the extent of my disease EXCEPT my trip to the cathe lab where the right coronary artery at the proximal location was 90% blocked. Even the doctors felt no stent was needed and were surprised once they got in there. I tell you I had no symptoms other than family history. I've lost 2 brothers already before 60 and my dad. My mom has had 2 heart attacks as well. I think my only ask of you is to not discourage the cathe lab so much as in my case it was legitimately needed. All your other advice is much appreciated and spot on - you've helped me tremendously as far as being an overlooked female with significant heart disease despite a very squeaky clean lifestyle. I was actually heading for a dirt nap. No one caught it and I was my own advocate. I wanted nothing to do with stents but nothing was going to reverse a 90% blockage but a stent. Thank you Dr. Brewer for all you do. I appreciate you and your crew for educating us but again, sometimes the cathe lab is the definitive answer and lifesaving...
With all due respect, CAC is potentially reversible based on patient’s reports-even at 90%-and has been known to spontaneously remit.
Dave Williams, a former Navy and airline pilot, wrote this:
“I am not engaged in any business that profits from the sale or Vitamin K2. But taking two brands of the Vitamin for 6 months (as MK7), two pills in the morning and two in the evening, my arteries and veins are clear of plaque. I no longer havean 85% blocked LAD; my LAD artery is clean. No plaque. I have had two Calcium Scores and both clearly show my two stents sitting all alone in a healthy LAD artery.”
He needed stents because he was admitted to the ER with acute cardiac symptoms.
I also have a paper titled , “Noninvasive and Invasive Demonstration of Spontaneous Regression of Coronary Heart Disease”, as well as a cardiologist’s report that a patient whose heart had been ravaged by myocarditis, not only recovering but imagery led the doctor to exclaim, “This looks like a new heart!”.
Not to belabor the point but whether it’s cancer, autoimmune disease, or heart disease, there are far more cases of the medically impossible happening than one would imagine. I now have a collection of more than 2,000 such cases and more are published each day.
Put simply, the body’s ability to self-repair is not only greater than we can believe, it’s more wondrous than we can imagine.
Why cant I see the answer?
So did you get a stent after all? Cause the message I am getting from this channel is that a blockage even that big does not need a stent if you are otherwise asymptomatic. Because, heart attacks can happen at any location with soft plaque of any thickness (not necessarily at the 90% blockage). So, unless it is approaching 100% blockage, a stent may not be needed.
@Daniel_Maxin - Yes on the stent. The blockage was soft plaque they identified as vulnerable to rupture - it was not calcified and was the surprise no one expected to find. I watched the angio and saw the artery blockage with my own eyes. There really was no choice in my mind that at age 55 I was not going to need a stent. I had a choice on the table and I chose stent. I didn't want it, but logic told me if not now, I was heading for a real problem based on the location of the blockage and strong family history of early deaths. If it had been 70% or less, I would have declined the stent. Too far gone for that.
@@sharon3750 Interesting. Thanks for answering. I did not know they can tell the plaque is soft or not on the angiogram. I have a stent myself but the angiogram report does not mention the consistency of the plaque (soft or hard) just the percentage (80% in my case) and reduced flow though I was asymptomatic. Location was bad though (widow maker ). But at the time I did not have the knowledge to question the stent recommendation and now I have it. Does not matter anymore but I still don't know if it was a right call or not. By the way...family history is also bad in my case as well.
Three years ago, a second-year resident, mentioned the word “widowmaker”. It was discovered during a “dual-source 64-slice CTA with 3-dimensional remodeling.”
I looked into it. The radiologist’s report said contrast was used. It wasn’t. Standard procedure is to scan only if the patient’s pulse is

Can plaque stabilize without a statin?
Great show...as usual, I am rivetted to your content...thank you so much
Overinflated like a blowfish. But other than that, it was ok.
congratulations!
My Doc. ordered the Fasting Insulin test 2 years ago, and again two weeks ago.
MY HOMA-IR score dropped from 2.2 to 1.3 which I credit to consuming plenty of Fish oil and raising my Omega 3 Index to 11%, which are not seen in communities outside Japan, Norway and the Intuit who consume traditional diets.
I read a long ago that consuming very little Omega 3 and at the same time consuming refined oils containing Omega-6 fatty acids can hasten Insulin Resistance by damaging the Insulin RECEPTORS.
And yes, I have a High Calcium Score, and I feel reassured by Dr. Brewer, although I would much rather that it were zero.
Don’t we all?
Warning ! This is not to be considered or confused for advise ! medical or otherwise !
If you might search/explore what was discovered about boron (alone) during the development of a boron containing compound for targeting the assembly of the
"NLRP 3 inflammasome" , ( crucial process in many inflammatory conditions)
*cellular in store levels of calcium began to lower moderately, but "not significantly" (at least in the context of the study drug's affect on the Nlrp3 inflammasome is concerned anyway) *-ca2 gradually lowered , and did not persist in dramatic lowering ...
I guessing , this affect was very important for "them" to nail down , after all, it could be found patentable or it may interfere with many other profitable medications affecting other conditions , like , for one, anti hypertensives .
So they twisted themselves into studious efforts to finally & clearly identify that the boron element being freed from their compound , became locally , cellularly available only prior to the breakdown/delivery of the other inflammasome targeting chemicals , that they also tested boron was not the component blocking the inflammasome . revealing that the boron element alone being freed was solely responsible for the subtle cellular
ca2 lowering effect .
To their extensive evaluation - a great many , > 1000's of other chemical compounds common or never manufactured were computationally explored for possibly reproducing this same affect , I presume for proprietary purposes , I'm sure , but to no avail ; left in frustration with a very marvelous result that could in no ways be monetized.
To my surprise , no media trending has yet taken up the usual reporting for even hype sake , as with so many other discoveries over benefits from natural remedies or nutrients , vitamins or other elements .
In their stating the following :
"Here, we use 2APB as a chemical scaffold to build a series of inhibitors, the NBC series, which inhibit the NLRP3 inflammasome in vitro and in vivo without affecting Ca2+ homeostasis.".......
&
"the main biological insight the use of NBC inhibitors led to was that NLRP3 inflammasome activation was independent of Ca2" ....
but , another point , not very highlighted is the discovery that elemental boron independently of pharmacological assistance lowered in-store accumulation of Ca2+ accumulation , in a "non significant" (to Nlrp3 focused study) manner .
but , they also , do not ever identify any cellularly disturbing, run away or damaging manner either , interesting , eh.
true they found many in their nbc compared series, that inhibited the NLRP 3 , but
without boron , but also under emphasized a novel conclusion.
was that consequently "boron" presence lowered Ca2+ in a not run away or out of control fashion . In light of this possibly leading to some population (large) no longer needing a Anti-hypertensive blood pressure medication ? WOW $$$ , a huge product use volume among those that most suffer with Nlrp3 related conditions .
I have suggested to my suspicion of boron's usefulness in parathyroid regulation but , I'm still puzzled by how it may lower Ca2 in store and have anecdotally some numbers of bio hacking elderly , gone off no longer needing BP, meds after experimental using .
yes , out here taking risks in life .. if ever get time to look into , anxiously curious ? what you might think ?
Many Many Thanks for your invaluable insights !!! Thanks to you Dr BREWER, my first step this Year is to have an OGTT ... Fingers crossed ...🤞
What is the best test for metabolic disease/diabetes?
I ordered the ultimate test for insulin resistance 20 years ago. I got a phonecall from the lab telling me insulin levels were unreliable. Then I retired and bought a horse. The insulin levels after a glucose load were not unreliable.
I bought pure confectionary glucose powder from amazon snd made my own drink and did the test myself at home. May not be perfect but it was close enough to check my reaction time and do the test.
How about adding lipid fraction and lp(a) to the interpretation?
I know the gold standard is ogtt with insulin, but is Nmr lipoprofile insulin resistance value better than Homa-IR?
So at a CAC score of 305 at age 76 and experiencing unstable angina, I guess I should be taking action. Maybe this video will help me decide where to go from here as far as testing goes. Can anyone recommend a doctor in Missouri that can help?
Oh, and the oldest settlement in North America goes back to 1500 BC in Mexico. Sure they were there long before the Europeans came to the continent but they were humans and shouldn't be ignored. Even in the United States it was in the year 650 CE with the settlement of Cahokia in Illinois. At the city's apex in 1100 CE the city covered about six square miles.
How do you know how many daily carbs you can eat?
A good general number is that Most people need to be at 50 or less net of whole foods - carbs from Veggies or berries NONE from processed foods... try to eat ZERO processed foods
Doc I went from lots of keto diet fatty foods for 2-3 months, and felt near as bad as when I had my triple bypass in 2015, ran to my cardiologist but then went on my own to beans/brown rice & Kale 7 times daily...and basically Dr. Caldwell Ettelstyn diet and felt near normal within 2-3 days!!! What do u think of that, I'm 72. Wanting to get a cac test next if I can. Good show sir. 👍
So, calcium test , any update on vit k2 to remove calcium from artery ?
Yes is that true or just a sales gimmick
Why no mention to couple the OGTT with C-peptide curve at 30,60, 120', 180'?
You can order many tests from Life Extension.
Have a research on prolonged water fasting and reduction of soft plaque using CIMT. 7days, 15 days, 30days, 45days.
Is this a regular blood calcium score?
I got an OGTT test a long time ago. I have never been so ill in my life when I got that test! They told me I was hypoglycemic-after using a CMG I know that I'm not! Also this video is difficult to follow. A bit too fluffy for a non-GP.
What's the protocol for oral glucose if the labs won't do a standardized test?
It's the continuous glucose monitor. That's the ultimate test?
In actual reality. You test yourself. You be dedicated.
So basically, if you don’t live in an area where there are practitioners familiar with metabolic health, educate yourself and implement lifestyle changes that promote metabolic health on your own. 🤷🏼♀️

How can we find a Doctor around where we live that is knowledgable about insulin resistance?
I would suggest you surf youtube for Doctors who seem to know what they are talking about...- you'll likely find one in your Region that you can at least drive to. Hint- they will be functional Drs who don't take Insurance but this is cheap stuff to test. Most are actually D.Os as they haven't been brain washed by the Big Pharma or med schools run by Big Pharma because they can't write Prescriptions and have to actually figure out health....and prevention. Most can even order bloodwork on phone consults. - curing Insulin resistance is pretty simple to reverse... Stop eating carbs and do intermittent fasting and it will be gone pretty quickly. Dr. Sten Ekberg in Cumming GA has great Videos on what to do... again he's a D.O. but much more knowledgeable.. for years than this channel. This guy - Dr. Brewer is very late to the game because he was coming from the wrong establishment and education.

An alternative to HOMA-IR is the Triglyceride-Glucose Index (TyG Index), which is calculated with fasting triglycerides and fasting glucose. TyG correlates with HOMA-IR. There are online TyG calculators.

And since we are assessing the highest score we may as well compare it to the age of those with the Lowest Score and the ages of those in the midrange who may be too active for their condition. Any comment or clarity is appreciated ..in advance.

@@PrevMedHealth Gracias Doc. Regards from Mexico.
Lovely video. Thank you! The one thing I would like to ask any doctor who claims APOB (or LDL) to be the cause of heart disease. The liver is manufacturing LDL in every human being from the time we are born to the time we die. We don't see a lot of babies die of heart disease. There are a lot of people with high LDL volume who never get heart disease. So would they still say that LDL is the primary cause. Is it possible that there is a different primary cause? Could insulin resistance be the missing link? Could LDL be trying to fix a problem with the arterial intima and the repair be plaque? If it is, would reducing LDL actually fix the issue or would it just prevent the fill up of the potholes and let the road keep getting worse till it collapsed? Answers to these questions may make the issue clearer to the general medical community.
Why bother with aCalcium test?
I got my fasting glucose and insulin level for the first time after working on low carb and intermittent fasting for a couple of years. Glucose was 99 and fasting insulin was 4.3. My problem is the ApoB and LDL are still pretty high. What should I worry about?
If your triglycerides/HDL ratio is less than 1 you might be alright.
Wish we could go to a training to read what is the numbers to manage with food
Inflammation is damage
Soft plaque is ongoung repair
Calcified plaque is repair done
I am not sure. Is soft plaque ongoing repair? If you are not living "right" the soft plaque could be repairing or staying the same. I thnk...

This "translate" joke is a bit tired...not to mention a real waste of additional time for no good reason. When you deal with something of such serious nature to some, if not to you, the "jokes" are downright annoying & only serve to dumb down your presentations making one wonder as to their legitimacy!
There are 3 purposes for the “tired” translations jokes:
1. We have many levels of understanding in the audience. It is an effort to provide repetition in simpler ways to emphasize & simplify key points;
2. There are many people that have stated they cannot understand sometimes due to medical terminology or Dr Vega’s accent. This is meant to help;
3. This is a difficult, technical, & emotional topic. We realize we are not the greatest comedians. But we do get a lot of positive comments about helping to lighten up the delivery of good, life-saving science.
