Yeah, Henry Townsend is such a creep.
I'm pretty sure that this manifestation and the accompanying sounds imply that the peep hole into his neighbour's bedroom isn't a fresh addition to his apartment at the start of the game and he's likely to have been using it to spy on her having sex and/or pleasuring herself when she thought she had privacy.
Stuff like this is where the series excelled, taking taboo issues and finding ways to get an emotionally charged response to what is implied through the environment, exposition documents, and dialogue without ever explicitly saying it, with the characters more likely to avoid directly addressing the subject, as if it's either too difficult for them to bring up, or they're putting on a front and never really being truthful about what they know or who they are.
It's a shame that the series didn't continue to leave so much up to speculation. I feel like the later games can be a bit too direct and would have benefitted from putting greater thought into using the environment as a means to give people an incomplete picture of what they want to communicate, but enough to come to a conclusion.

We know the main character spied on her alone in her room. My interpretation is that since he was spying on her extremely private moments (for example - masturbation), he feels shame and the face is judgmental while getting revenge by watching him.

There is no other way to determine it its definitely sex moaning. Its supposed to be alagory to peeping on her through the hole to her room then that idea flipped on its head literally with her staring point blank at you in a very creepy and sexual manner basically saying you should feel bad and creeped out for spying on her. Trust me this wasnt some random cheap jump scare this was put in there on purpose for a reason. Pretty effective and clever

SH4 may not be the best ever, but this was one trully disturbing moment that has just stuck with me for years. The only thing that kinda ruins it is that you get to come back to this same room with Eileen and she has nothing to say about it. "Ow it's just a room with a giant version of my head, cool"

I don't know if I agree with that interpretation.
(Disclaimer: this is my reading of this scene and you're entitled to feel differently. There are no direct answers to specifically say that the below is the case, however I do think that there's sufficient implication present within the game to support my reading. Don't take this as definitive, just consider it a possible alternative)
I feel that Henry is more of a stalker, as implied by the peephole that opens into her bedroom.
My inference is that this is a reversal of the voyeuristic dynamic that defines their real life relationship, before all of the supernatural events began happening.
The sex sounds to me imply that Henry has been looking in on her bedroom for libidinous reasons.
We don't know much about Henry but things like this, and the lack of any significant response to his disappearance, could imply some fairly dark implications about his life prior to the start of the game when it comes to how he relates to other people.
