Regarding the yacht anchor...thats not unusual at all. When you're dealing with a ship that large the strain the anchor is under is immense. And with all the other factors sometimes you just put too much on a piece of steel that might have had an unknown defect which happens. The anchor room/winch on any ship is known as one of if not the most dangerous places.
