Please I’ve seen some care plans and some write the self care in present and others in past tense. Please which is it? Should self care be written in present tense or past tense? Thanks so much.

Hello, I just washed your video on assessment now, you said if the patient has 3 in single parameters in NEWS you can write 1 and someone will not fail , they failed me and that is the reason am re- sitting the exam again .

I thought for GCS you don’t need to check pupils and muscle strength or is GCS and neurological examination the same ? Cos you did both . If told to do GCS ,are we to perform both?

Pre operative care plan.
Nursing problem/need: pain due to ruptured appendicitis with a pain score of 7/10.
Aims if care: Hailey Brooke will verbalise less pain with a pin score of between 0-3.
Revaluation date: Hailey Brooke is to be reevaluated every hour and as clinical condition changes.
Care provided
1) Explain all aspects of care to Hailey and gain consent for all interventions.
2)monitor Hailey’s vital signs hourly and escalate as per NEWS 2 policy.
3) Asses Hailey level of pain hourly using the pain assessment tool.
4)Teach Hailey deep breathing exercise,reposition for comfort and offer diversional activities.
5) Administer prescribed medications to Hailey and monitor its effectiveness after 30minutes.
6)Refer Hailey to pain management team or surgical team for further evaluation.
7) inform Hailey to use the call bell.
8)Document all aspects of care rendered and medication administered.
Patient self care.
1) Hailey verbalised understanding and participated in her plan of care.

Aims of care : Hailey will not show signs of haemorrhage during hospitalisation.
Revaluation date : To be reevaluated every 15 minutes for the first 1 hour ,thereafter every 30 minutes for the next 1 hour,then hourly for the next 4 hours and as clinical condition changes .
Care provided by nurse
1) Explain all aspects of care to Hailey and gain consent for all interventions.
2) monitor Haileys vitals signs every 15 minutes for the 1st one hour ,thereafter every 30 minutes for the next 1 hours and hourly for the next 4 hours . Escalate as per NEWS 2 policy
3) Assess Hailey for signs of bleeding
4) Teach Hailey signs of bleeding
5) Inform Hailey to avoid strainous activities
6) Teach Hailey deep breathing excercise and repositioning to aid comfort
7) administer prescribed medication ,document in drug sheet and monitor its effectiveness after 30 minutes
8) inform Hailey to use the call bell
Refer Hailey to surgical team for further evaluation
9) Document all aspects of care rendered

Why do you always say we should use scale 2, even when patient is in or off oxygen.
My love for your osce exam use scale 1 and neva scale 2,scale 2 is for copd patients
What if you have copd patient? Would you still use scale 1 or 2?
If you have copd patients ask your examiner,but I doubt, is always scale 1 for osce
Please do we do A-E assessment in a patient with MUST. I didn't see you mention that. You only did observations
