A couple of comments:
1. This was the 'happy ending' US version of the movie. The original UK version is different. After Sarah throws up, turns around and screams after seeingJuno in the car, the movie cuts back to the cave. Sarah is still there and is staring with a smile at her kid's birthday cake. The camera pans out and you see that she's really looking at the flame of her torch, as the creatures are coming in. In other words, she hallucinated escaping and has 'descended' into madness. The movie then ends and we're left with the knowledge that it's only going to be a moment or two before she gets killed.
2. While you're angry at Sarah for impaling Juno in the knee with the pick-axe, you seemed not to understand that Juno had been having an affair with Sarah's husband, Paul. Look at the way Juno looks at him at the beginning of the film, then secondly, as Beth is dying she hands Sarah the necklace she grabbed from Juno's neck. It has Paul's favourite saying on it, indicating that he gave it to Juno. When Sarah sees this she knows about the affair and then later, when she asks Juno if she waited till Beth died, Juno lies and says, "yes". So that's two strikes against her. Pluuuuuus, it's Juno's fault they were all in the wrong cave in the first place. So think about all of this when you're considering if Sarah over-reacted.
