Yes, the added height will change the aero somewhat. MPG will decline somewhat with the lift/large tire combo

Anyone buying a truck isn’t considering obtaining 38 mpg. Anyone lifting a truck isn’t concerned with reducing the mpg even more. Very moot point you brought up. Lifting and leveling a truck is about visual aesthetics. Not being green.
