    Nurses learn how to assess a patient's ability to read and compare that to a normal average adult. Using the Snellen chart, nurses will position their patients 20 feet away from the chart and instruct them through a variety of tests where both the right and left eyes are assessed separately and then both together to determine the patient's visual acuity.
    There are many reasons why a patient will need a visual acuity exam completed. There may be complaints of irritation or signs of infection, injury, or declining function. This is a baseline exam that may result in a referral to an optometrist or ophthalmologist for further assessment.
    Understanding how the Snellen chart is graded will help nurses accurately educate and document their findings.
    In this video, we discuss how to set up your Snellen chart, patient education, and instruction for administering the exam and assigning a value and documentation.

    20/20 is 0.9 cm
    20/40 is 1.8 cm
    All proportional.
    This is
    1 minute of arc,
    and 2 minutes of arc.

      20 feet is the focal point for distance if the test is done closer the distance results will be incorrect if the test room is not large enough a reversed Snellen chart should be used viewed through a Ophthalmic Viewing Mirror with the distance from the patients eyes to the mirror added to the reflected distance to the chart both distances added together should equal 20 foot

      That distance is what a person with a normal visual acuity could see those corresponding letters from. For example a person with normal acuity could see the letters well on the 5th row from 20ft(6m).

      20/20 is normal and not in need of eyewear, anything else signifies a change in vision that may need investigating (depending on patient presenting issues and history of natural visual changes)

    Every optometrist has an eye chart with a big, "E", on top. Just for once, I would like to have my eyes examined by a doctor who has, "biggies", on the top of his eye chart. Just a nice pair, and I would prefer them in artistic relief, like sticking out from the wall, not just a picture. And, they would have to be pert, not like, "National Geographic", biggies, as they would hang down so low, that they would block any reading of the lines of letters below them. IMHO, any optometrist who would use such a chart must be declared to be incompetent because it would be likely, eventually, that they would misdiagnose a Chinese lady as having a cataract, when she really owns a Rinkon Contenento.

      Snellen char make a required 5 nodal point 5 minute of arc basis but this board letter looking is very small 6/6 letter size is required 8 mm height and same like width
