    Melasma is a common skin problem with high recurring nature which is characterised by formation of light brown to dark brown or greyish brown patches on the skin. It occurs more commonly in women and most of women develop it during or after pregnancy. It is more common in dark skin people. Common age group is 28-38 years.
    Triggering factors for overproduction of melanin which cause melasma:
    Major factors
    -Hormonal factors (Pregnancy, hormonal therapies, oral contraceptives)
    -Exposure to sunlight (Ultraviolet and possibly visible light too)
    -Family history
    -Skin phototype (Darker skin is more prone)
    Minor factors
    -Thyroid disorders
    -Stress
    -Cosmetic abuse
    Medicines photosensitizing drugs and anticonvulsants
    -Iron or Zinc deficiency
    -In men - Low testesterone levels has been observed.
    Symptoms- 3 types -
    Epidermal- means superficial pigmentation, light brown in color
    Dermal- Means Deep in skin, greyish brown in color
    Mixed- present at both layers, dark brown to grey brown in color
    Sites-
    Face- Cheeks, Nose, Forehead, Lips, Jaw line
    Extrafacial- Forearm, Arm, Neck
    Diagnosis - Mostly made by Clinical Examination only.
    Treatment
    Melasma can fade on its own or persist for years if left untreated. It tends to come back even after it has faded. Treatments aim towards fading the pigmentation, but none can guarantee that it won’t come back. Melasma treatment is based on combination approach for getting best results.
    Medical treatment of melasma (Sunscreen, Creams or oral tablets) is first step for that.
    Patients who do not respond well to medical treatment or someone want faster results chemical peels and laser treatments can be combined. It takes 4-5 months to achieve satisfactory improvement, there after maintenance treatment follows.
