How do u overcome overlap of MCA branches , to safely microcatheterise distal Occlusions?
There is usually overlap of MCA branches , how do you superselectively cannulate the occluded distal branch ? ( We can't have a road map from M1 due to overlap)
Any ASPECTS cut off ? Because you don't want to give Anti-platelets in patients with low Aspects , as it increases the chance of bleeding
What's your Anti-platelet proctol for doing carotid Stenting in this setting?
IV ASS load with 500mg CT next day and if ok load with Clopidogrel
The middle meningeal looks small , did you use sceptor mean still ? Why no magic microcatheter?
Magic is also a great choice! I guess the scepter mini gives you more security regarding reflux that’s why we chose it 🙂

Great technique! But it means I have to use a small stentretriever for a large vessel due to the small pusher wire needed, right?
It can also be used with a 4mm Preset Lite because of the thinner pusher wire Or you go smaller with a 3mm Trevo, Preset, Solitaire etc that also have a smaller pusher wire

My challenge with distal Occlusions is overlap of MCA branches, how do you overcome overlap so that u safely microcatheterise distally?
Series from the M1 with a big AC helps Or you can do a 3D DSA and overlap with 3D roadmap
When you are in pain finding the right branch do a 3D dsa which is btw saves radiation compared with multiple noninformative 2d aquisitions

It is called pORTAL EXT extension wire with a length of 115cm. Both wires have a diameter of 0.014" / 0.36mm. Thank you for your interest!

good job guys!! interesting the removing of the aspiration catheter with subsequent new advancement, while the stentriever remains in place. A question: don’t you aspirate when deflating the flow- gate?
Sure we always do distal and proximal aspiration. In this case the ot burden was so large that the flowgate was clogged!
