Manual Blood Pressure Physiology
A Blood Pressure (BP) measurement in its simplest form is a determinant of individual cardiac output (the volume of blood pumped out of the heart and into the aorta per minute) and the systematic vascular resistance (the diameter of the arterial blood vessels)
Many factors can influence a BP reading, for example, age, sleep, emotions and activity. Consequently, evidence in the literature varies as to what constitutes a normal reading, and optimal parameters are often used instead.
The normal BP at rest has the range of 110-140 mmHg for systolic and 70-80 mmHg for diastolic BP. Hypertension (high BP) refers to measurements that exceed the resting systolic parameters and hypotension (low BP) refers to those that fall below resting systolic parameters.
The manual auscultation technique is considered the gold standard for BP measurement, advances in medical technology have resulted in alternative ways of measuring and monitoring BP.
Moreover, while electronic devices have become the ‘go to method’, it is essential that healthcare professionals maintain the skills and confidence in using the manual method, especially for patients with hypertension, hypotension or pulse irregularities. Pulse irregularities can cause inaccuracies in electronic readings and this can lead to false diagnosis and incorrect treatment.
When taking a blood pressure it’s important to listen for Korotkoff sounds which are generated when a blood pressure cuff changes the flow of blood through the artery. These sounds are heard through either a stethoscope or a doppler that is placed distal to the blood pressure cuff.
Phase 1 is a sharp tapping sound and indicates the systolic blood pressure. Phase 2 is a swishing sound associated with blood flow. Phase 3 transitions to a softer thump, while Phase 4 is a thump that becomes muffled. The diastolic blood pressure is determined when all sound ceases at Phase 5.

    From my understanding I think the patient has had an exacerbation of asthma caused potentially by an anxiety /panic attack which may have caused change in breathing… etc Administer nebulised bronchodilators and monitor to see if wheeze has cleared and if oxygen saturations are within normal range. If no change continue to administer nebulised bronchodilators and reassess.. considering transport to A&E for further examination.

      Strong hypothesis and i can say that your management definitely helps to treat certain symptoms, as her condition has exacerbated her asthma, what sort of bronchodilators would you give considering her present symptoms ? We also need to think about the presenting complaint which includes increased weakness in limbs, headaches and neck stiffness, what could be causing those symptoms ? Also think about the history of presenting complaint with her just getting over Covid, could that be causing something more sinister, more neurological in nature ?

      @@SkillfulResponder0304 I would administer nebulised ipratropium to help with their wheeze on 6-8lpm, and that due to the nature of the medical emergency with the limb weakness, neck stiffness, and headache transport to the nearest care facility could be beneficial. Queering some sort of post covid inflammatory syndrome which is having an impact on the patients nervous system with the differential being a transient ischaemic attack stroke

      Nebulised Ipatropium is good call with the patient already using their nebuliser at home and it not having much effect and although there are no contraindications in giving salbutamol in an emergency situation, it’s always good to be cautious giving salbutamol when a patient is tachycardic and hypertensive and this could qualify as a severe case of their asthma. The ipatropium helps ease the bilateral expiratory wheeze,and you load the patient on to the ambulance, and the patient is now on 91% saturations with nebulised oxygen, the nebuliser has now finished completing its medication. What else can be used to provide a more efficient amount of oxygen delivery ?
