Well thank you for the question Dr. Medical Management alone for such patients is futile in my opinion. I am guessing, we are discussing about the presence of the thrombus scenario in a patient. The reason is that in this particular case- the thrombus is well organized and "hard" (not exactly 'soft' as you might put it. Such type of thrombus do poorly with medical management alone and take a lot of time to clear, if they clear at all, that is. In a patient with ongoing chest pain, and no distal flow, with a not-soft thrombus, there is limited scope of medical treatment alone, and patient would require PCI asap, like in this case, to protect the myocardium as much as possible. About intracoronary tirofiban, I like tirofiban, and some use cangrelor, well, it all depends on a lot of factors. About the triple therapy, in this case, the patient was only kept on DAPT after adequate treatment with PCI. Triple therapy may theoretically sound better, but it subjects the patient to a massive amount of risk for bleeding. But yes, if the thrombus was soft, and the flow was non-limiting and patient pain free at the time, i might have gone with the approach that you were talking about. But in this case. The only right option was this one due to all the factors mentioned above.

Uttarakhand is a beautiful place and maybe some roots of mine are there, but yes, I love uttarakhand, especially Kainchi Dham, Neem Karoli Baba and I visit there every year atleast once. But to answer your question, not that I know of.

Guidelines are there to guide you not dictate all the real world practices that you do. We are a Radial First Center, however, this patient had an extremely tortuous Radial Artery course that would have taken precious time to negotiate and would have hampered the door to balloon time hence a switch over to femoral access was warranted to save precious muscle. We do Radial/ Ulnar wherever feasible as the first thing and only switch over to femoral when required.

Sometimes, when there is a CTO (Chronic total occlusion), which I am suspecting in your case, then there may be a need for rotablation, however, as a technology, I am not very fond of rota, and like to utilize other modalities at hand. However, having said that, sometimes, Rota does become a necessity. Also, I think you are confusing "catheter" with a different thing that the doctor might have said, which is very common to do, since it is nearly impossible not to be able to use catheter- the access site may have to be changed, but all angioplasties and stenting are done through catheter only. Anyways, my follow up question is - Has the RCA been opened (the blockage relieved)? by rota or by anything else? Also, do you have any other blockages in your remaining two arteries, if the RCA disease has been relieved.
