How I bring up weightloss to patients
zhlédnutí 405Před 2 lety
This is how I present weight loss to patients. First, I ask if they think weight may impact their hip or knee pain. If they say yes, then I guide them through the common pitfalls and focus them on the important issues of setting a goal, picking only one strategy, and tracking how consistently they follow that single new behavior (NOT their weight). If they stick to the new behavior, joint pain ...

    Does this work on tendinosis, and if so, how would this address tissue inside the tendon and not just the outside? I.e. collagen fibres that are not aligned and in a mess.

    With hEDS (hyper mobility) we know tendons don’t heal with PT. They can only heal if injected an irritant like PRP/Prolotherapy or Stem Cell therapy. Had PRP done in many areas and I can finally walk, sit and stand without pain. I am no longer having subluxation of my joints thanks to PRP.

    I loved this explanation, thank you! I noticed you talked about the outcomes for ligaments and tendons, but what about other aspects of a joint when it comes to PRP? The humerus of my shoulder is slightly raised (subluxed) when I treated desk work like a sport for one week. I had a desk that was too high for me that caught up with me by Friday when I went too intensely on a project. Fast forward, it's now been a year of shoulder impingement that's very gradually improved. I also have mild subacromial bursitis. My shoulder becomes retracted in the afternoon, some mornings I will wake up and the joint is fully mobile. I can't do yoga, do a full body stretch, desk work aggravates it, sleeping is more complicated, etc. Do you think PRP might offer some benefit to bring a raised humerus down into the capsule so to speak? I had a doctor recommend it recently as an option. I read on Johns Hopkins it can increase joint mobility, which you addressed in your video in relation to the knee.

      Wow. This sounds exactly like the issue I am dealing with. Been dealing with an impingement for the better of 8 years now and have been there 6 different PT facilities and two cortisone shots and nothing has ever worked. Yesterday my doc recommended PRP due to my chronic issues. My ball of my shoulder is slightly raised as well.

    Hello Doctor, I have three questions as I explore Tenex 1. I have a lot of pain in the top of the forearm, which I don't think is all from the TE 50% epicondyle tear. Will Tenex also "investigate" the forearm area? Can it explore only the forearm area? I had an MRI but hand-ortho, did not identify any issue there. I suspect scar tissue there too, maybe some neve entrapment that fires only upon dynamic use. Will the ultrasound during Tenex help figure out: 2. My Ortho says TE tear surgery has a very low success rate. Does not recommend Tenex (I assume at the tear) because it will complicate the surgery if I do end up going there. What are you thoughts on that? 3. I have some forearm pain (kind of like golper's pain but not at the median epicondyle, weird). Also, constant non dynamic pain in the elbow crease, in the area of Tendon of biceps brachhii. Perhaps right where the Median Nerve crosses the elbow crease. MRI shows nothing. What do you recommend, ultrasound?

    Thanks a lot for your video Doc. I've been having chronic pain on my wrist and had a initial TFCC injury which led to it healing after a cortisone injection but an eventual ECU Tendinopathy. I got a PRP injection into my tendon but the area appears to be slightly swollen and I haven't got too much benefit out of it. I wonder if another PRP is need or just time? Because its been upto a year and I still havent seen an elimination of pain yet....

    Thanks for the great and honest info! Having had achilles tendonitis ( I am a RRT and often help RNs lift heavy patients- free workout!), but strained my tendon a few times by going up on my toes). I have OA from 12 yrs of sprinting and am a research nerd. While looking for articular cartilage repair( Chondrofiller etc), I came across some research where nitro patches were placed on affected tendon areas to increase blood flow and No3 to the area. I got a prescription from an ER doc who seemed interested and achieved good results. At 63, I started javelin throwing as stress relief and exercise, my knees feel terrible when I try to run, do javelin seemed less stressful and I plan on competing again. I developed golfers elbow because I forgot my age and over did it, but the nitro patches worked again. I would suggest this ad an adjunct therapy for tendon issues. I used .4 mg Nitro-Dur patch 12 hours a day and moved sites each time with good results! Peter Hild RRT ( knees are ok when walking, plan on working until 70. My sprint training came in handy, never lost a race to a code blue in 30 yrs!!)

    • @jaguarreal9116
      @jaguarreal9116 Před rokem

      @@Jack-Surreal_Panes I'm sorry to hear about your other elbow. Maybe it can be addressed less invasively than the 75% tear. Are both elbow issues the same? Partial tear? By the way where do you feel pain on top of the forearm when you manipulate just about anything with finger tips. I get that pain even with a TE brace. I'm wondering if the tendons in the forearm are also too tight, and/or compressing the nerve. I have been researching doctors like crazy. It seems that few doctors actually do elbow (even though in their profile) because of the low success ratio. You got lucky to get ten years out that elbow. What state are you in? I am thinking of trying an initial tele health with a couple of doctors, one actually in Iowa, a couple in New Yort, one in Boston, just to see what their approach might be. My tear is at 50%, so in that no-mans-land of just keep waiting, try PT, try injection, try life modification. Tenex popped up, but my doctor's facial expression was negative, "it helps some people, but it will complicate a future surgery, even too many injections complicate a future surgery". It is exhausting, so in some ways just knowing you have no other choice is kind of a relief.
