Prosthetics & Suction Suspension Systems
Suction is one of the simplest suspension options for prosthetics. For general durability, suction is hard to beat, but the outer sleeve adds weight, and restricts movement due to the extra material. The sleeves also have to be replaced regularly because they'll eventually wear out and get a tiny hole, and even a pin-size hle means they won't be able to maintain suction which results in a pistoning effect with your socket and residual limb.
While most prosthetic supplies like liners can be replaced via insurance every 6 months, sleeves are generally eligible to be replaced whenever they're needed. Adding a gaiter or inner sleeve to go betwen the socket and outer sleeve can help reduce friction and extend the life of your sleeves, but at some point they will still wear out. In a pinch, duct tape can be a short-term solution to fix a small hole, but it will quickly wear out. For me, that's meant needing to bring more spare parts on trips when I relied on suction. If a sleeve got a hole in it mid-trip, I'd be in a tough spot without a backup, and since they're thick rubber, they definitely add extra weight when packing.
In my experience, one of the biggest benefits of suction systems is the overal fit and security. As long as there isn't a hole in your sleeve, they stay on snugly. However, due to rolling the sleeve up and down, suction systems are the most difficult to quickly take on and off. It's not horrible, but it's definitely not as simple as with a pin system.
For what it's worth, I've started using a sleeve with my hybrid pin/vacuum system so my leg is currently taking advantage of all three types of suspension. While most of my old sleeves have holes in them, the vacuum system helps ensure that's not a significant problem like it would be with pure suction. Using all three types means more time to take it on and off, but it's the most secure system I've ever used by far.

    This is great 👌🏻here’s a few tips that may help - My prosthetist made some pelite foam wedges to help with getting the alignment right with different shoes heights so definitely take those shoes with you to your appointments. wrapping your footshell with a plastic bag or cling film/plastic wrap will help shoes and boots slide on easier. I added a pool noodle to pad out the pylon and adapters from wrecking my snowboarding boots and the plastic bag trick works amazing on snowboard boots 😉 I use velcro to hold my flip flops (just make sure the hook side is on the flip flop and the fluffy side is on the foot shell or havianas stock strapped flip flops. The proflex align is a fantastic adjustable foot for those that like to wear heels Hope this may help 🦿🤗

    Hi, have you tried botox treatment on your stump? I have, and my experience it works very well on sweating on your stump. What it does is that you sweat much less. The treatment you do every 4th month.

    I ride English and just had a zipper put in the front of my tall boots. Commonly they have zippers in the back these days but they didn't work well for me. Ariat just released a line of western boots with Zippers especially for amputees. I wear the Ossur ankle that works for wearing heels because it also works for putting one's heel down to ride. Medicare has disallowed it but I've sent in an appeal.

    It's been a couple of years since you made this video, hope things are going well for you. I have been thinking about having such a leg. I had foot surgery 4 years ago and I have been in constant pain and discomfort. Surgeon can figure out why but what you explained has been on my mind just to get some relief..good video, stay well!

    I’m an RBK for 7 years, started with a pin lock then went to a simple vacuum. Time for a new leg. I’m thinking of a socket with an assisted vacuum it’s a Rush foot. Have any other suggestions?

    Very nice video. I'm trying to fine-tune my socket and Proflex XC foot. My immediate issue is getting the socket to fit as well as possible. I have the A/P correct, it feels great with equal pressure all along the anterior of the stump. The socket still presses too much medial at the top of the socket. Not sure if the adjustment for this is best found at the bottom or the top pyramid. I'm experimenting & I'll figure it out. Thanks for the info you shared, it helps me understand.

    Hi, i want to thank you for putting these video out, its very informative and ez to understand. i currently have the same system with my first leg, and my leg has shurnk too much that I have to get a new sucket. i have a short leg limb below knee like you do mine is about 4 inches. and I just watched your hybrid vacuum and pin socket suspension system video, can you tell me which one would you prefer? this or the hybrid? if you got the time to reply my comment I will really appreciate that!!!

    Below Knee Amputee for now 60 years, One of the Leg Accessories [Sleeve] I cannot use, it causes me ENORMOUS Skin related issues, better to go to a Vacuum or Pin suspension & eliminate the sleeve & especially in HOT Weather.

    I have lived with a crushed ankle for over 30 years and had it fused 20 year ago. Contemplating BKA. Trying to learn. . .I have thought about it off and on for 20 years. . . And the thought of running again would be awesome. If people have "warnings" I'm open to hearing.

    I just got a prototype prosthetic leg to use before I get my permanent one. are you supposed to feel pressure on the lower inside of your knee when you use it? sometimes I feel pressure & if I use it for a little bit it starts to feel like pain. also, I can feel it hyperextend my knee at times.

    Lbk 40 years now. I have a viscous ball for torque, and a multi axis ankle. Carbon fibre foot, split toe. Socket is fibre glass with an ACL generation 2 brace integrated into the socket. Insurance through work (retired now at 62) pays for everything. To be totally honest, socket design is the bane of my existence. Osteointegration just isn’t for me, esthetically ..I just can’t accept it and structurally , including load profiles, it just seems like bone failures could become the norm. Leakage around the connection on the distal end of the residual limb would be unacceptable in intimate situations. I apologize to anyone who feels otherwise. Again, some sort of pneumatic or liquid filled liner adapting dynamically to changes in weight and/ or residual limb variations in dimensions would make it a perfect socket fit and would be golden to me!

    I’ve got a question I’m really curious about. Is it possible to wear, like, knee-high boots with a prosthetic leg? Since most have a side-zip, getting them actually on might not be too difficult, but would the actual height of the boot be an issue? And would it make a difference depending on whether the boot was also lace-up or not?

    Well I’ve had my prosaic for around 6 months now a lot of the time I’ve had trouble getting in and out of the bathtub so I just kept using those packs of wet wash cloths like they use in the hospital they are made of coconut fiber so instead of just tossing them in the trash just wash them out and reuse them they last a long time you can also get the coconut fiber in rolls like paper towels they are a little bigger. Anyway I was getting broken out from the stuff on them several years ago I was having problems with my skin and also smelling bad I was doing a lot of heavy work outside with machinery and was bundled up usually with 2 to 3 layers of Levi’s pants and jackets and would sweat quite a bit during the day the thing I found that worked the best was apple cider vinegar I fill up a spray bottle around 1/4 to 1/3 of vinegar then fill the rest with water. After a bath or shower I will spray under my arms and between my legs with this it eliminated all of the complaints about me smelling bad and none of the problems I had had with deodorants plus it is a anti-fungal and kills bacteria after losing my leg I will spray the inside of my liner and wipe it down with the vinegar spray and do the same with my stump also a couple of times a week I will just use the coconut towels with about a tablespoon full of rubbing alcohol on it and wipe down my liner and stump which they told me to do a couple of times a week anyway. I haven’t found any of those socks you were talking about but when it’s been in the 90’s and above I’ve been wearing my Shrinker sock underneath the silicon liner it’s a little tight but I don’t seem to hardly sweat this way instead of a half cup of sweat it is barely damp when I take off my prosthetics and it dries in just a few minutes way different than having to mop up all the puddles then trying to put your leg back on again. Give the vinegar water a try it doesn’t mess up your skin it’s cheep and the smell of the vinegar is gone in a couple of minutes way faster than the deodorants and they’ve been using this for thousands of years I was surprised to find this out I’m guessing you can’t sell deodorants if you tell people how people use to do thing’s before in France they would just spray perfume on but nobody else did that 😄👍

      Get yourself an extended shower chair. It goes over the side of the tub & is quite easy & safe to use. Your insurance should cover the cost.
