This is not the first time for Alexios to visit Athens, he came before, for him this is the second visit. So he might also be the reason for plaque. But not only Alexios or phoibe but also many people might came before them. In the beginning story Phoibe asked Alexios to save her friend and his family they were in plaque affected area. the priest were trying to burn the family. Alexios didnt save them, they might affect other people. But in alternate choice Alexios will kill the priest and save the family but then in later story when he will vist Athens and saw its worst condition he will regret his earlier choice.

@@Gamegamesgamer that’s what happened in my experience with Alexios. I killed the priest, saved the family, and later on Athens was plagued. Phoibe was killed:/
