The one thing I don't understand is why they played Russian roulette. The Smoker was the only one with a weapon, seemingly neither Fritz nor Soldier were tied or cuffed to the table. Nothing was stopping them from turning the gun on the Smoker and slamming the trigger until it fired at him. ...also why the hell does Fritz's smile at the end look so uncomfortable?

Late to the party but didn't realize till now. Fritz 100% killed Jeremy's mom. That Respawn machine is known to glitch with our proof being Monster Scout and The Hunter still having one leg leading him to replace it with a metal rod. So with Fritz dying at the end with no eyes and what looks like no teeth, The Respawn Machine didn't fix them. That's why when Fritz saw himself in the nightmare sequence killing Jeremy's mom, He was missing a few teeth now and instead of replacing his eyes, The Respawn Machine was trying to seal the eye sockets, that's why there's bits of skin poking out, or a much darker thought, it did seal his eyes and Fritz had to cut them open to "See" correctly. Edit: This might also Explain Dell and his brothers. They must have lost their eyes at some point and The Respawn Machine sealed the eye sockets up. which also means that Dell and his brothers had their eyes destroyed so much, they either stopped caring in "Replacing" the eyes themselves or the goggles they wear work as a way for them to see again.
