      Great question! Technically, pedicle screws are meant for temporary fixation while a fusion is healing. However, they can become painful or unnecessary and can be removed.       Thank you so much for the comment! Our ORs are used for both spinal and extremity surgeries. A majority of our spinal surgeries are done anterior (or through the front) so a Jackson Table is not required.

    Well done Sir. Thank you for this very informative video. Have always had pain in the lower back after 3 fusions. The surgery was helpful and I'm grateful for the relief but there is still persistent pain. I'm thinking the screws, I have had a recent Cat scan was told the fusions are solid. Going for the top two lumbar fusions later this year,hopefully we can get a better look at the screws.

    My mom had a surgery on her l4 l5 prior to which she had constant severe pain on her left leg .. after this surgery in 2 weeks she has severe pain on her right leg and she could hardly stand up . She's bedridden . Could it be this?

      Failed back surgery syndrome could be a possibility for your mom. Severe pain lasting that long does need to be looked at by a doctor and new imaging of her back might help assess her condition. We do offer free MRI reviews if you have a recent MRI post-op.       Screw move and wiggle, hurt where nerve live. Take screw away and pain no hurty. Got to give it a little bzzzz to make sure first. If bzzzz says no, then we no go. I think.

    How long should residual nerve pain last after lumbar microdiscectomy? Is it normal to still have residual nerve pain 3-6 months after the microdiscectomy surgery?

      Hi David, thank you so much for your questions! It is unusual for nerve pain to last 3-6 months after a microdiscectomy. This indicates that a post-op contrast MRI should be conducted to see if residual compression is present. We do MRIs on site at our Atlanta and Savannah locations, but we can also do e-visits if you send us your imaging. Wishing you all the best on your healing journey!

    Thank you for sharing a great video. Very worried after very bad fall. Very bad button pain groin, throughs can’t walk far doing my head in. Waiting now. Had leminectomy surgery some 50 ago. Fractures of T section from fall also need to lay down most of the time. I have faith in a Nuro I have seen some 4 times now just need to think positive but hard at 72 not young

    I had Harrington rods placed in my back from T4-L4 when I was about 9yo (I'm 40 now) and I've had so much pain that has been progressing every year. I've just figured that this is my life and I'll always be in pain. If there anything new that could be done to help?I'd love to speak with you if possible. I have double major idiopathic scoliosis and prune belly syndrome.

      Hey Holly, it is possible you might be still experiencing pain due to your hardware. We would recommend a CT scan to check to see if your fusions were solid which could give the possibility of removing your instrumentation.     Had fusion of l4-l5 spondylolisthesis with bone graft and hardware 33 years ago. Been suffering with symptoms of neurogenic claudication since 2019. Had a decompression of l2-l3which failed. Symptoms have slowly worsened to point of being quite miserable now. One problem is that the hardware doesn’t allow imaging to really see what’s going on which has prevented me from getting any help.

You are correct about the hardware that it does make it hard for imaging, however you could get a CT myelogram (not an MRI) and that would show more clarity to your particular case.       My entire lumbar spine is fused. Two ALIF/PLIF. One XLIF/PLIF. Of course the micro and laminectomy. Multiple ESI, Caudal and Transforaminal injections. I had a DCS implanted in 2021 and a hardware revision prior to that. Dec 23 we fused the R SI Joint and this Friday we’re doing the left. So far the SIJ has given me incredible relief and I’m hopeful that the left side will be it for awhile. I’m only 60 but have advanced osteoporosis, stenosis, degenerative disc disease, etc. The next failure point is going to be T12-L1. It’s already quite diseased but I’m going to put that fusion off as long as possible. I was 29 when I started down this path. I have a fantastic spine surgeon who has done all my procedures except for the SIJ fusions. We have a great spine hospital in Oklahoma City so I feel blessed.

    I did microdiscectomy 3 years ago l4 l5 but doctor removes l4 disc only now i did micro decompression and discectomy l4 l5 l5 and s1 pain was now at my right leg was very hard i can't even sleep if i straight my right leg the pain was increased

FBSS doesn't always indicate surgical failure; it may stem from pre-existing conditions, non-surgical factors, or the natural progression of the spinal condition. Thorough preoperative evaluations and proper patient selection enhance outcomes. While acknowledging risks, dismissing spine surgery based on FBSS oversimplifies a complex field. Decisions should be collaborative, considering individual circumstances and the evidence supporting specific interventions.

    I had a major cervical spine surgery in December of 2021 I left with 20mg oxycodone and haven't been able to go down 😢I'm in constant pain 24/7 I can barely move

    I was fused in 2018. In 2020 due to continuing pain we decided to pursue a Dorsal Column Stimulator. To my dismay a hardware revision (removal) was suggested and a second opinion agreed. I was VERY surprised how big and painful this was. I didn’t anticipate such a large incision and several days of hospitalization. I drained more than was expected and had to be kept longer. I just want to warn everyone that it’s not just a simple thing to open you up and remove hardware. I’ve had two A/P fusions so the anterior plates and screws are obviously still in place. I can highly recommend a DCS (Dorsal Column Stimulator) to anyone who is still experiencing pain into their legs. It’s not magic but my opioid use has decreased.

      Yep, relieving severe pain isn’t really even a goal anymore and largely taken out of the equation. ‘Suck it up buttercup and you really should learn to cope better’. It all seems to get flipped back on the sufferers. It’s brutal and you are not alone in this.

    I'm having all the symptoms of a loose screw on my L5-S1 fusion but xray at the ER show nothing. Would a CT scan be more accurate?

      Yes, a CT scan would be able to show more detail than an X-ray especially if you are experiencing lucency in your screws.

      It can definitely be painful to have a herniated disc and bone spurs in the disc space because of it. Luckily, this patient came to see us and isn't experiencing any pain!

    I've had too back surgeries and know I have crps1 and chronic pain syndrome on my right lower leg and as of 12-20-2023 I had a spinal cord stimulator permanent procedure done to help with the pain life is different for me know

    I have failed back surgery syndrome and now rely on 70 tablets and morphine each day. It's so severe I can't tell when I have kidney infections or anything else wrong. I've had 2 spinal surgeries, my spine is degenerative and I've has serious issues with it. I couldn't walk for 3yrs, l4/l5 ruptured and completely crushed the sciatic nerve all that time, I was on a nuero waiting list. I'm diagnosed as having severe periphial nerve damage and sciatic damage. I was 24, doc didn't believe me for a year because in his words "I was too young for there to be anything seriously wrong". This needs to stop, mine wasn't caused by an accident or trauma, it just happened. I have 7 or 8 things wrong with my spine now that are all comorbidities of the original problem. I can't be fused and will only get worst. I have a very destructive type of arthritis throughout the spine, the level of damage I had they said they'd only seen in major trauma, all mine starting irritating the cauda equina and I'm left with permanent damage from the waste down. I don't feel much in sense of touch, I feel numb alot, have horrific parathesias of different sensations, my hips and spine lock up and my legs are very weak. My neurosurgeon ill be forever thankful for.

    I got extreme nerve pain in my contralateral leg after my spinal fusion surgery. It is better now but i still hzve a numb leg. My spine has not fused. Is this a sign that my surgeon stuffed up? My surgery was supposed to take 2 hours, but it took 5. He said my nerve compression wasn't too bad, so why did i get such bad pain after fusion?

      Unfortunately, that is a possibility. You may be experiencing pain from a possible malposition in the pedicle screw. We recommend getting an evaluation and a CT scan to get a better look at your particular case.

    Exactly right with having pain. I knew after 4 months something was wrong. However, it took another 12 months to get a CT scan which showed none Union, a pedal fracture and 2 loose pedicle screws.

    Hello Doctor, my mom had laminectomy with 2 level discoidectomy for L4 -5 ( severe) and L5 S 1 ( mild) . After surgery she walked and become stable for few days but after almost in second week she is feeling shooting unbearable pain at her thigh left side. Type of pain - back side of left thigh and pain is up to knee it last for few seconds and she ask us to hold the pain full area but our hand so that she can bear this pain. Is it failed surgery? Note : before surgery she had pain in both legs and it was for last 20 years and area was only left 2 mm ( normally it should be 13 mm).

      Yes, that would be a sign of failed back surgery syndrome. It would be best for her to get a consult with a follow-up MRI.
