Paramount Television (2003) [HQ]

From "Star Trek: Enterprise" starting with season 2 episode 13.
The 2002 Paramount logo was visually updated and the 90th Anniversary text was removed. For those who have seen my Star Trek: Voyager and Frasier logo uploads, you may be familiar with how this fanfare sounds, seeing as it's the same one heard in some of the videos from the aforementioned shows (here, it's not cut off at the start). It would appear that this is where they may have sourced the Paramount logo used to plaster the older logos on those shows.
