Medical Scribe Clinical Scenario - Example Medical Chart Documentation

Follow along in real time with a medical scribe and observe the documentation of a complete SOAP note for a patient presenting with a migraine. The viewer will hear dialogue between the doctor and patient as the video shows the clinical scribe documenting the visit. In this example clinical scenario, you will see the documentation for subjective and objective patient data including the HPI, ROS, Physical exam (PEx), Assessment and Plan.
