As anguish said - 4 strokes anywhere with the hands followed by two kicks, should be in double triplet speed ("sixlets"). Listen to any Slipknot song for examples, they use them all over the place.

I would say most guys I have worked with nowadays have this approach. They have a double pedal and use only one kick and the other kick drum is just for the looks. The feeling of playing is the same but it's easier for the sound engineer. It also gives flexibility in the placement of drums because you don't actually have to be able to reach the other kick drum.
