When I do it, it correctly grabs the extra parts of the hair that I want, but for some reason, the opacity of what is added is different. With the white background, it looks like someone spray painted the added parts white.
Great tutorial. Have struggled with this big time, not any more! :-)
Figured it out finally.
After using Refine Radius Tool and making girl's edge hairs appear. We have to adjust 'shift edge' bar in order to bring back original hair color. Perhaps you need to point this out before selecting output ' layer with mask' . Thanks all the same.

Bob, please help. I used the same picture using same refine edge tool. And I got girl's hair is gray color, not original color. Don't know why this happens...please help!!!
