I started with my CPAP about 2.5 months ago. It's been a terrible experience. I have only recently come across this channel. The information here is so good that it makes me angry that I was provided any of it. Tonight will be my first night with SleepHQ, and the information it provides will be so helpful. Why my Dr doesn't have this level of information is beyond me.
The paradigm of finding the right mask is madening. There is no test fitting before purchase. The idea is that you are supposed to keep buying masks until you find one that works is asinine.
My fingers are crossed that I'll be able to sort it out myself.

Videos like this would be great to get sleep doctor's and technicians to watch. The doctors around my area believe they know it all and scoff at people who try to learn how to improve their comfort and aih. They also try to get us to ZeRo events and keep us there and I'm not a fan of doing a sleeping marathon of terror each night. This will be my second go around with CPAP therapy and I'm going to insist on low pressure settings initially as ramping up to quickly will just lead to me not using it at all like before. I'm going for my second sleep study but this time with CPAP so I really need to keep an eye on the techs ramping up pressure too quickly.

Mine is IT Training, since 1982 and without wishing to appear a 'know it all' I have sussed out the use and reading of my Oscar data but on asking this clinician , what I would consider low level questions i.e. "Now getting use to a full face mask, not taking breaks during night from mask any more, no more panic attacks, gone from 3.6 hours average per night to 7.3 hours with keeping mask on, slowly increasing my pressure and humidifier settings - Why has my AHI averages over four months not changed at all?" I’m sitting here, very happy with your video and really unhappy with my pulmonologist. I had an AHI of 9 at my sleep study but I have mild pulmonary hypertension and use 02 at night.
He recommended a CPAP to reduce general fatigue. We discussed setting it at 8. I used my machine and it was a shitshow. Uncomfortable, mouth breathing and dry mouth. Switched to a full face mask and then had a very upset belly and pain. Called Doc and pressure turned down to 9.
Still have Aerophagia plus with the full face mask it’s hot and giving me fits with nightmares of suffocating. Saw your video today. MIND BLOWN!!!
I called my doc and asked for another pressure reduction. I am keeping my full face mask as it’s expensive and I’ve already ordered a nose pillow mask on my own. I’ll use the cpap again when the pressure is reduced and when I get the nose mask. I strongly suspect it might actually help me.
I’m strongly peeved because I’ve had to learn all this ON MY OWN. Considering what my pulmo makes per appointment, I would have expected more support. I’ve learned more from YOU in a few videos during the past two weeks then I learned from my physician. Thank you.

I totally understand.... had a similar experience with my sleep study/ CPAP prescription. Pulmonologist never actually saw me, I only dealt with the sleep techs. The CPAP would cause the inside of my nose to swell up and I would wake with a completely blocked nose and/or sore throat. My AHI was only 15 during my sleep study, mostly due to sleeping on my back and less than 5 when on my side. The CPAP brought me down to 1-4 AHI but I could only sleep 1-4 hours until it woke me up.
I've been getting a lot better and uninterrupted sleep since I returned it. Thankfully here in the U.S. the insurance companies force the equipment providers to lease CPAP machines out and I don't have to pay for a machine that I am not using any more. Going to see if EPAP/ Provent might work for me. Hope you have better luck with yours. :)

Thanks for this breakdown of the SleepHQ results. I accessed the clinical settings via your instructions on my AirCurve machine. A few days later the clinician phoned and I was scolded because the BIPAP pressure they set the machine at was 22/12 and I had turned it down. "This is a prescription. You shouldn't change it", was what I was told. So, I had been doing quite well at a lower pressure, but they did not want that. So, I've been fiddling around with different mask types and watching reviews from you about different full face masks. I haven't touched the clinical pressure settings in a couple of weeks but my results have been varied because I was trying to find a mask that worked for me. My AirTouch worked okay, but started to leak around my eyes when up to full pressure. I just got a F&P Evora Full Face mask and have been wearing it for three days. NO leaks. When I slept I had results under 5 events per hour and it feels pretty comfortable. Sorry for the long-winded comment, but I have really been inspired by your channel to work on my sleep apnea and try to get it under control so I can sleep better, lose weight, and be more healthy.
Thanks so much for your channel and the work you are doing with this app to help us CPAPers. I know I can get this under control again and make therapy start working for me.

Increasing medical evidence apparently strengthens the link between untreated sleep apnoea and stroke. 3 yrs ago, my bro at 59yrs old had a devastating, massive BrainStem stroke and survived. In the absence of other risk factors untreated sleep apnoea was thought to be the smoking gun. Taking CPAP therapy seriously and doing your best to make it work is so important. I have had CPAP for over 12 yrs for really severe OSA (AHI 80 and SAO2 75%) & probably wouldn't be alive today without it. G'day mates, there's an 'Easter Egg' but I've made it much harder to find. If you can find it I'll send you a 02 Ring & SleepHQ hat If you find it, comment on this thread and let me know A. Where you found it & B. Who They Are (there's a clue!)
The no.1 reason people give up on CPAP therapy is terrible advice and education. The 3 fundamental elements of success are
    1. What therapy mode? CPAP, APAP or BiPAP
    2. What pressure settings provide COMFORT & CONTROL
    3. What CPAP Mask
These 3 elements are linked and provide the foundation for CPAP compliance. Don't expect your clinician to get it right! They're not the ones sleeping with the mask. In fact, most have probably never slept with CPAP.
Today's case study was just an example of how you can work on the 3 elements to improve your CPAP results. It is not a 'one size fit's all approach', and the methods we used to improve Jays' results might not suit you.

      FIRSTLY
Well how the fact on learning I would have to wear CPAP mask for the rest of life, that caused me to wake 6-7 times a night, a blocked nose so had to war a full face one, a skin itching condition that even wearing a watch was a problem, found wearing the mask a very big problem.
Did a lot of research on the Internet not just CZcams and found VERY LITTLE in regard to any alternatives but primarily mainly obvious commercial backed ones for wearing masks.
By pure chance came across a study carried out in Japan that losing weight, exercising and a far better diet if serious enough could lead to not having to wear one.
Thus, for a year using the discipline I learnt as a young man in the RAF learning Tae Kwon Do, most days of the week I did about 2.5 hours of exercising: indoor rowing, cycling, dumb-bells,, kettlebells, punch bag, and out door running and power walks. Totally changed my diet and went from 134kg to 92kg which for about a year now have kept. Saw a CPAP specialist who got me to carry out the same overnight sleep test and to her amazement could not believe I had absolutely no signs of sleep apnoea at all. I also only wake one or twice now, not having to wear the mask.
      SECONDLY
Been teaching and training IT since 1982 and thus know very know what is required to produce You Tubes, which are to all intents and purposes 'training videos' which require a lot of time, energy, effort and most of all money, i.e. decent kit. Having well recall the very amateurish and non-profit-making efforts of the early days of CZcams, I can assure you that what we see now are often better than major film and TV companies would be able to produce and there are many such companies that offer such services to CZcamsrs, plus the obvious financial support from companies to assist CZcamsrs who have the gall to say they are Independent of the company they are doing a review on.
      MY POINT
You are simply, as with so many people now are being very naive, trusting and incredibly and dare I say, STUPID because if you think I am being unfair in my comment to Adam by stating an alternative to wearing a CPAP mask ( albeit one not many could do) as well as my actual experience in the of IT and the Internet - you still have the mind of a child and believe in Father Christmas and Fairies.

Nick you are the best. I am 35 years old, overweight, about 265lbs. Been getting terrible sleep for 5+ years.
Finally got a sleep study done, my AHI is super high. Severe OSA.
I just got my first CPAP machine less than 60 days ago. And... Sadly... And very unfortunately... I honestly have not been able to get a single minute of sleep with it.
Typically, and for my entire life, it's always been an ongoing joke (with family, friends, and lovely better-half of 18 years) that I can sleep anywhere, even on a hard floor without a pillow.
I typically fall asleep within 5min of laying down, when tired.
But...... Add a CPAP into the situation (brand new AirSense unit)... And BAM! Somehow, no clue why, my body/brain is physically absolutely incapable of falling asleep.
I've tried probably 25 different nights over the past 2 months.... and not a single minute of recorded sleeo.
If I leave the mask on, I literally am awake nonstop... And... In the event that I get close to "dozing off", I often wake up feeling panicked and pounding heart, as if I was being strangled. (Oddly enough.... Very very different from my typical apnea events).
I was given zero help from the horrid facility/team that I got my CPAP from...
They immediately set my settings to AUTO mode, with a min of 5 and a max of 15, and said "that's standard, and what most people use"...
I've tried pressure relief settings, levels 1, 2, and 3.
    I've tried pressure changes.
Tried lowered my max to 13. Then 11.
I have a full face mask... Which the CPAP company told me was a REQUIREMENT because one of my nostrils is fairly collapsed, so I typically breathe about 90% out of my mouth (especially when sleeping).
    Am I lost cause?
Am I a case that cannot succeed with CPAP treatment?
I really want to feel well rested for the first time in my life.
I've been dieting and exercising so much for the past 8 months, and struggling to lose weight, as I never rest properly.
(I'd love to be your next direct patient, that you link up with, and help me find success with this therapy that I want to work more than anything in the world. I want my life back. And I want to stop sleeping in the guest room, because my middle of the night choking scares the sh*t out of my fiancee!)

I am simply just tracking my calories, and keeping myself at a proper limit, based on my desired goal weight. And I also force myself to walk 8k-12k steps every day (whenever possible). And the weight is just falling off still!
APNEA HAS ALMOST RESOLVED ITSELF. Not fully, obviously, as some of it is my anatomical structures in my nose and throat.

I have used a CPAP machine for a lot of years. Initially I had a sleep study done. When I had the appointment to get the results from that the specialist said that my results were so bad that he didn't want we making the hour plus journey (country roads AND city traffic) to get the sleep study done with CPAP at his recommended settings. Instead I was set-up with a machine on hire and sent home to try it out. The most difficult task was the trial and error over the next couple of months, to get a mask that I was comfortable with. I ended up with the nasal pillows, which I had originally discounted because of my hay fever. I think that the mask (Swift FX) has helped with that. I wear a chin strap to stop the mask leaks but the bug-bear with that is the velcro ripping my hair . . . . so I put something over my hair (I have settled on a toweling hair turban/towel), so that is no longer a problem. After watching your video, I thought I would have a look at what the read-outs were on my machine. Instead of have realised that I cannot even turn it off. For the past few years I have simply plugged the machine into a power board with individual on/off switches, and simple turn the machine on and off at that switch. (Very necessary due to back pain when twisting further . . . and I sure don't miss having to put my glasses on to read the screen.) Perhaps my machine never had those read-outs, just the mask-fit screen and then it moves on to auto. What machine do I have? Wait for it . . . It is the ResMed S8 Auto Set Spirit 11. It works . . . I wake up rested . . . the Fitness app on my watch indicates that I have an amazing deep sleep vs light sleep percentage (40 - 50% deep sleep) and, if I have to get up during the night, it indicates that I usually fall back asleep very quickly. I tend to 'roll with the punches' with what life throws at me . . . . When my dentist told me that my teeth were getting worn down because I must grind them in my sleep, I thought, "Oh good god! Now I have to wear a mouth guard!" Truth is, that isn't so bad. I didn't go the expense of a custom piece from the dentist . . . . and I found that the shop-bought mouth guards are too rigid and perhaps a bit too high on the gums. I figured that this was because they were aimed at impact protection when playing sports.. Thank goodness for eBay and finding mouthguards aimed at just stopping teeth grinding. They cost less than $2 each. You pop it in hot water for 20 - 30 secs, take it out to cool slightly before you press it onto your teeth and the roof of the mouth to shape it to fit you. If you aren't happy with the result, let cool and then repeat this procedure . . . A bit of advice: Don't get the clear ones . . . particularly if you have bad eyesight. They are very had to find, when they have rolled somewhere across the floor or gotten caught up in the bedding!!

Everyone told me how great cpap was but I’ve been so upset. Im going to get the new mask on Monday. The same as Jay here in the video. I’ve been using a nasal mask and I often get nose irritation so that I have to stop therapy for a few days. I do use a chin strap but I think I do have a lot of mouth leak anyway. The place that sold me my equip said that I can’t buy a new mask for three months because my insurance would not pay. It’s been three and it’s time to change. I don’t have a card reader yet but as soon as it comes I can get my data uploaded and see what is going on. Thank you again!

I was unexpectedly hospitalized away from home and the hospital provided me with a CPAP for the night's stay. A PT set the machine up asking me about the ramp and pressures but we did not account for the filter they use to isolate me from the hospital's machine causing a restriction of the flow rate.
When I gave her the pressures and told her that I don't use a ramp, that I needed the pressure right away to fall asleep, she was surprised. Later that evening, I felt like I had to suck to draw enough air so I opened the settings, cranked things up to where I was comfortable and fell asleep just fine.
In the AM, the PT came by and I mentioned that I had to turn it up because of the filter. She blew a fuse; first being surprised that I knew how to do that and then a little unhappy that I 'trepassed' on her territory. I explained that I've been on CPAP for a long time and that, IMO, CPAP users better know how to adjust the machine themselves, intimating that they just can't run to a PT every time they needed to adjust their machines. [ I know some will always be uncomfortable changing settings but most should know how.]
Thank you for your channel - I am satisfied that I am getting the most I can with the settings I am using because your content is helping me understand what the readings mean and how to fine tune my machine. For example, as you were describing the patient's index numbers, I immediately knew he had to go to a full face mask or try to stay under the burst pressure for his mouth leak.
PS - I also think that you do a truly professional job with your media presentation - it is a real stand out in the CZcams universe!

@@generalismoquabablinotheth5902 When it is working for me, I generally know immediately the next morning. If you are not feeling better, large leak throughout the night could be an issue. A properly fitted full face mask will generally fix large leak. Also, you might not have the correct pressure(s) dialed in yet. Does your machine provide a data file for you to examine? This will tell you if your mask is leaking. Some machines will beep at you when there is a leak. Is the machine auto-adjusting (adjusting pressures in real-time as your body needs support)? I am nowhere as bad you Jay, (not rubbing it in, really sir) just saying that we all can learn from this example. I switched from a nasal to a full mask when my seasonal nasal passage swelling and plugged nose hit in the fall. My previously successful trial and first few weeks of my own machine went down the toilet. I ordered a F & P Evora full and could breathe again. But at 2 am, each night, my pressure would increase (set for max 12) and I woke up with my mask farting away. This vid helped me to reduce my upper pressure to a minimum and I am gradually increasing the upper limit. So far so good and I got 8 hrs, 20 minutes of good sleep last light with a high of 6.2 pressure. I will stay there for a few days and up it gradually until I get unacceptable leask rates.

    You videos are really helpful.
Can you add a feature on SleepHQ which lets us know what should the best pressure setting be based on our previous data?

I cannot get My mask to seal prperly and I snug up the mask so much it hurts and thus 4 hours is the max that I can wear it. I have a pacemaker and can't wear the mask that I found comfortable because of the magnets, the mask connected to the hose at the top of my head and it was comfortable because, I am a side sleeper. I would rather not wear the mask at all if I had to try sleeping on My back. I am 77 and I don't mess with the settings of the machine because, I don't trust myself in remembering what the hell I did last. I have to tell you that all the stuff you are presenting just totally scrambles My mind.

I have been om CPAP (APAP) for 7 months. But the first 5 months i was using a nasal mask and it did not work, now i am using a pillow
mask and chin strap which works great. My AHI is between 0,5 to 2.5 and when i was tested it was 68!

I had high Blood Pressure. Cpap use helped to lower it,. but Airsense 11 keep telling me about leaks. I don't control the Resmed. It is done remotely. I can't afford mistakes with insurance or settings. I have had a stroke so comprehension is difficult. I need to become an expert to continue? I never thought of YT -U would help. Glad I found your channel.

I've been a CPAP user for almost 2 months... and your channel has been a godsend. Thank you.
I read comments, (somewhere among your videos), one man kept saying he would turn off/on the machine.
Two things have directly been impacted by your videos is that,
    1) ordered a nasal cushion mask and
    2) I turn off the machine and turn it back on because I start bubbling air out of my mouth.
What I have realized is that when I fall asleep and my jaw drops back, I end up mouth breathing and air leakage and my pressure goes up. And when my pressure went up too high, I would have to turn off the machine and turn it back on so that it would reset and ramp back up. It allowed me to go back to sleep.
I originally had a full mask and I loved the feeling of it because it equaled the pressure between my nose and mouth. But I could never find a good position for my tossing and turning side to side that I didn't have constant air leaks. And again I wasn't even on the system a month and it was a struggle. So this nasal cushion has done wonders for my sleep, though I still wake up to the noise and the leakage and discomfort. It is way better.
This video proves my own hypothesis. After I got off the full face mask and gone to the nasal cushion, my air leakage mouth breathing improved because I sleep side to side and I turn off the machine because otherwise my mouth bubbles air.
Thank you for this very much.

I really appreciate how this answered one of my questions regarding leak rates! I have a beard and I have had high leak rates and when you mentioned it, I felt relief. I was thinking I was doing something wrong! I'd like to know of options to mitigate this without having to shave or possible change masks.

I would encourage new CPAP users to try different mask models, not just one model of each type, until they find a mask they love. Also, try masks that have been out for years, not just the "latest and greatest". In my experience mask design in full-face masks has gotten worse for me, not better. Seems like manufacturers are more interested in planned obsolescence than patient health.
Your hat's on backward. Try turning it inside out. ;)

  • @philosophicallyspeaking6463

This....'variability' of result, relative to equipment, is great for people selling these products. Is there any 'legitimate' unbiased statistical 'generalized' preference for machines, masks or pressure that might define a reasonable place to start. It is very telling that the machines are sold without masks of any kind despite the fact that the manufacturers also sponsor or manufacture the masks as well. For what it costs to manufacture these masks, manufacturers should include examples of each with these unnecessarily expensive machines (as 'medical' devices, they have a ridiculously high markup relative to general consumer goods).

I personally found this really depressing. First he had to keep on increasing his pressure. Then he had to switch to a bulkier mask. Then he had to shave off his beard. It just sounds like his experience is getting worse and worse.

Sorry for the late response, I just noticed your comment. It hasn't't been as bad as you think. The F30i just works for me. Nicko's intervention happened at exactly the right time, with zero leaks from the day I shaved. But a little thought and some judicious trimming as the foliage grew back and now I have a beard the looks better and has just enough of a trench that there is no fur under the mask. I'm used to solving complex software problems, and understand that some things take time and patience to work out.

I'm angry at the CPAP equipment retailer and the hospital sleep centre. I've had a great AHI but leak well above 30 even when I mouth tape. I've bought several different masks and chin straps. I watched an earlier video on this channel and dropped my max pressure to 10 and my leak was 0! I went back to the n30i and my leak is between 0 and 10L.
I battled leak for a year and no one suggested I change pressure settings. I tried max pressure 9 for one night but my AHI rose to 1.
Now I just have to decide if I want to return the P30i while I still can. I like it's minimal touch but it's made my skin bleed, and with reduced pressure my p30i is working.
Good on Jay, but it shouldn't be up to patients to educate themselves and feel slightly naughty making their own changes to their therapy. I have an upcoming medical review and am worried I'll get in trouble for changing my pressure settings from 4-15 to 6-10., even though now my AHI and leak rate are great.

I was ready to give up. I hated being air-raped and just couldn’t tolerate it. My tech tweaked the settings snd now I sleep comfortably with the CPAP. My AHI went from 140/minute to 3.5/minute.
