Ok, i found the name of the instrument and it is made of goat skin. It is called
Ney-anban (or Mashkak)
Ney-anbān is a type of bagpipe which is popular in southern Iran, especially around Bushehr. The term ney-anban literally means “bag pipe” but more specifically can refer to a type of droneless double-chantered bagpipes played in Southern Iran. This is similar to the Bahrainian jirba played by ethnic Iranians in the Persian Gulf islands

The clip shows a collection of authentic Persian instruments. 3:10 the instrument is Ney Anbaan. The closest thing in the English language to it would be pipebag.

That instrument you said is like a balloon is made of i think goat or sheep skin in whole, and the mouth piece he's blowing in it is actually installed in the front leg of the goat. You can actually see the shape of the animal if you pause the video. Unfortunately i don't remember the name of the instrument. I will add it if i could remember it.
