    I would guess that to adjust to the RB splitting out into the slot the LBs would just drop into zone, but what if the #2 split out into the slot?
    Or would the defense just change from the cover 4 completely?

      Zohraish Virani no whomever splits out, whether the TE or the back, you read it from outside in keying #2. So instead of playing cover 2 on that side, you also play cover 4

    So if the #2 (TE in this case) is staying home to block, what's the reason for not having your safety help the Corner a little more on the fade?

      It is because the Safety is a run-first defender, and by the time he reads his key and diagnoses play action pass, it isn't likely he would be able to make it all the way past the numbers to help on a fade. If there was a post, he could drift back and rob the #1 receiver, but the way the actual play action is designed it purposely makes it hard on the rest of the defensive backs to help on the only route they are really running on this play. They have to read run, which takes them a hitch or two, and that pulls them too far out of position.
