It should be decreased as there can be fluid loss and in case of fluid loss our body considers Lithium as Sodium ions. So it may cause toxicity. It's just like taking Li in fasting like dehydrated states.

In 3rd trimester, the dose should be increased due to increased renal clearance and when close to labor, it should be decreased
