  • Why are some people having thrombocytopenia after COVID vaccine? The reason isn't clear, but I can show you how platelets work to stop bleeding and what are the four main causes of thrombocytopenia.     I also found a personal application. Many years ago, because of an unexpected medication interaction between a newly prescribed med and a common OTC, I had drug induced thrombocytopenia. I got a panic levels, emergency call from the lab, after I had the blood draw and was told to stop all meds and go directly to my MD or the emergency department. Thrombocytopenia is usually not a panic situation, but in rare cases it can be life threatening. Thank the Lord for conscientious lab technicians and lab directors. And doctors that take unexpected bruising seriously.

    I really wish that I had access to this video last July when I was in the hospital for 10 day(4 of which were in the ICU) because I got hit by ITP out of the blue and had a platelet level of 2(2,000). It would have made things so much more clear than not understanding the multiple hematologists attempting to explain to me my new condition. Job well done on a basic primer.

      @@friendlyneighborhoodimmuno7192 Actually I have been stable at around 100,000 for nearly 6 months after 4 rounds of Rituximab last July/August and weekly Nplate injections until the end of last November. Honestly I dont even think about it anymore. My body doesnt feel the same but maybe thats age...and I DID get the Covid Vaccine(Pfizer) with my second dose a month and a half ago with no negative reactions...note to fellow ITP'ers

      They diagnosed me at my 12 week pregnancy check and I was in hospital for most of the pregnancy. They took my spleen out and now I'm in the most vulnerable group. I'm pleased to hear that you are doing well

    I just told my wife I could watch these videos all day Informative and making me recall my college Anatomy and Physiology classes ... If I have to Change my current career, I will head back for immunology ... Thank you for sharing this. Sent this to my son who was diagnosed with ITP and almost had his spleen removed. He asked his doctor to have his vitamin D levels checked ... Doctor laughed at his request ... Doc had to eat crow when she called him up saying you need to go to the pharmacy and pick up a prescription for 50K Vitamin D Rx ... His ITP seemingly went into remission ...     You have such a great voice! Great information - thank you. My 11 yr old son was diagnosed with acute ITP on new years. His platelets were 15 10*9/L. He received IVIG. Due to vomiting/headaches from tx he was in the hospital for 4 days. His platelets on recent blood test were 460… too high now. Has anyone experienced this? Is there a chance that he won’t relapse? He only had a cold prior to this happening. He did have one dose of Pfizer, and has had covid 4 times…

      Sorry to hear that you have ITP. Sometimes ITP can be genetic but most likely it’s due to a viral infection. When your body fights off certain viruses the protein your B cells react to is similar to a protein in platelets. Type I diabetes is similar in pathology. The Virus looks like beta pancreatic cells or it infects pancreatic cells and in the immune carnage the B cells accidentally make antibodies to the cells of the pancreas. I hope your case is mild and manageable! Take care!

    Incredible class on thrombocytopenia. Good review of nearly 40 yo Anatomy & Physiology, and Pathophysiology courses for me. Your videos in general are very informative, better than almost all similar on CZcams videos by MDs and wannabes. You are performing a vital service.
    You failed to get into the specifics of DIC, And why the usual standard of care for thrombocytopenia is contraindicated for many cases of thrombocytopenia that is sometimes a very rare AE of the J&J/Jansen vaccine and in DIC.
    I guess it might be confusing to introduce at this basic level, because the mechanism is counterintuitive, But it does help explain why the initial cases were detected in women by an alert Ob-Gyn, as peripartum DIC is always "on their radar".

    Might be a bit late, but I had thrombocytopenia as a result of the Covid vaccine,a day after the first jab. Hospital team saved my life with prednisone, yet my body has never recovered back to a normal life, with symptoms very very similar to long Covid. Reported to the ministry of health, but never received any support.

    My first dose of astrezeneca gave me ITP (first dose was about 2 months ago), so far ive had medications and 1 immunoglobulin transfusion, is this temporary of chronic?

    Very informative❤explained alot to me. I never get sick, when I get cut I stop bleeding fast, don't have headaches. The blood test show I have low platelets. I do bruise easily my doctor says I have thin skin is why I'm bruising I'm in my 70s. How does milk products play a part in my low platelets. I mainly eat cheese, milk, homemade yogurt ,eggs, pound and half of nuts a week,all greens, vegetables,alot of fruit,very little bread, no beef,a little chicken and mackerel.❤❤

    After my vaccine i had a blood pressure cuff leave nasty petatchie red marks on my arm that didn't heal for a week. My platelets were normal range, so what could cause that? I've had really slow healing dermatitis from a heart monitor too.

    I had less than 1 platelet and was bleeding from everywhere. Never been more frightened in my whole life. Doctors thought it was cancer at first but it was covid. Luckily it worked with high doses of prednisone which made it rise to like 60 within a week

    Great Lecture. I am a physician that has had thrombocytopenia for over 20 yrs.The etiology is still a mystery. I hovered around 40K . 3 bone marrows over the years have not revealed any myelodysplastic features except an MCV of 113. After the Covid 19 Pfizer vaccine a routine check showed my platelets were down to 17K. I got a bunch of immune studies that did not show any platelet antibodies. I got steroids with were only minimally helpful. Now getting Rituxan which I hope will help. Any thoughts? You cam PM me

      Thanks for the comment. I am sorry to hear you have had to battle with thrombocytopenia for so many years. It is interesting that you don't have antibodies for platelets, and yet the treatment chosen is to further suppress the immune system instead of boost megakarocytes (Romiplostim)? Does your physician suspect ITP? I'll PM you more ideas soon.

    My platelets have been hovering around a 100 over the last 5 years. Not really sure what caused the drop - could this be a side effect of certain common antibiotics? The only trigger I could think of. My antibody blood tests came back normal, and blood marrow seems to be healthy. Yet, i have not been able to bring the levels back up. In addition, I have a Factor V Leiden single gene mutation (prone to clots), and Type A blood type - not a fun combo to have during Covid! Have my 1st dose vaccine next week, followed by a blood test 2 days out and 12 days out. I’d appreciate any tips! And thanks for the video - very helpful!

      Hi Yelena! Do you mean 100 or 100K? It seems like based on your tests that your doctors have not found immune cell involvement. Perhaps there is a genetic reason behind your low platelet count? It sounds like you have made a smart plan with your doctors. Good job taking care of yourself! I think the two blood tests and monitoring your symptoms is the smartest thing to do. I don't have any specific advice for boosting platelets, but relaxing, eating well (fish, olive oil, avocados etc), spending time in nature are all immune boosting activities. I wish you the best!

    I am so scared to get the vaccine because of this, I have ITP only when I’m pregnant but even if I’m not pregnant my platelets tend to stay on the lower side. Is there anything that helps boost your platelets besides a transfusion and steroids.

    As a low platelet person, I found your explanation very informative. I was prescribed Furosemide which I took for about 6 weeks until another routine check of my blood revealed platelet count of 50K. It has been low for sometime since I have an autoimmune liver disease, PBC, but my Dr. was alarmed and told me to cut down or stop taking the diuretic which has the side effect of lowering platelet counts. Trying to live with this, I recently retired (at 70) to go at a slower pace and dedicate time to my health.

      @@friendlyneighborhoodimmuno7192 my brother has liver cirrhosis. He wakes up sometimes with blood in his mouth. He takes Lactolose, Xifaxin, furosemide, lasix, . The doctors have never said anything about duretics cause low platelets count. Is this true?. They give him duretics so he didn't accumulate water retention on his stomach lining . He's had to have water drained off his stomach twice in last 8yrs. I loved your video. Thank you for your hard work. My brother took the jab against his doctor's advice and the rest off our family won't do it. To many people getting blood clots and adverse reactions. Doctor the work you do is very very important in the medical field we need more like you. Blessings to you and your family.

I am not an expert on liver diseases, but I did check out each of the medications to see if they were associated with low platelets. Xifaxin seems to be very helpful during liver cirrhosis: pubmed.ncbi.nlm.nih.gov/22098272/

      Thank you for the links. My 2 sisters have pbc caused i think by hepatitis, not alcohol abuse. I have now developed low platelet count the most recent fbc showed 96 for thrombocytes. Gp is reluctant to refer me to haematologist at what he considers to be an early stage. My feeling is 'why wait?'. The condition has been monitored for >12 months and shows a steady downward trend. Receiving zero treatment, only monitoring. Why wait?

      Hi Sathish, I’m sorry to hear you have thrombocytopenia. I always say to consult with your doctor before making health decisions, but I will also give you my opinion. It seems like you have mild thrombocytopenia, but I would recommend getting your platelet levels checked after your first vaccine shot. Covishield is a viral carrier vaccine meaning that a DNA virus from a chimpanzee will bring the information about how to make COVID antibodies into your cells. Covaxin is an inactivated COVID virus. No active virus in Covaxin.

    My 9 year old brother is currently experiencing thrombocytopenia. That in addition to neutropenia as well. His platelet count was less than 3000 recently and they havent been able to find a cause. He is not symptomatic at all but he had bruises all over his body and whenever platelet count drops below 10000 he develops petechiae on his feet. His gums have bled a few times too. They did a blood culture and bone marrow biopsy and found nothing out of the ordinary really. The only treatments that help temporarily are platelet transfusions and IVIGs... they might start him up on a low dosage of sirolimus medication soon.
    This video really helped me understand better... A job well done teaching. Do you suspect thrombocytopenia and neutropenia in his case could be associated with the COVID 19 vaccine (he got his second dose in January) in his case doctor?

    Several days after the 2nd Pfizer shot my wife presented with petechiae, nose bleeds and bruises over her body.
    Her platelets, neutrophils, lymphocytes and RBCs were way below the reference ranges.
    She was Hospitalized in emergency for 10 days, released and then reinstated to emergency 2 days later for another 5 days.
    After evey test under the sun including 2 bone marrow biopsies because they thought it was cancer stemming from the bone marrow, neck and armpit biopsies, bone scans and on , she was finally told it was an autoimmune attack her doctors were and are suspicious of it being vaccine induced.
    3 months later she goes toothed hospital once a week for blood tests. Her platelets are going down again. With IVIG and steroids her blood products mentioned above wen from platelets at 4 to 250 , neutrophils from .6 to 3 and lymphocytes into the middle of the range. Platelets are now at 60 and dropping. She is likely going to get treated with rituximab this week.
    It was a terrible time for her and the family. We hope her next treatment improves the condition.

    Hello there. Would this same concept apply to your annual flu shot as well? And if so, then can someone with ITP in the ranges of 100k-150k that has taken the flu shot for several years now and done well with their counts, expect a similar result with the covid vaccine?       Good question. It would be encouraging to know that the flu shot has not affected your ITP. I would always check with your primary care doctor about ITP and vaccination. It does seem likely that you would experience something similar with the COVID vaccine. You can set up blood draws one week after each shot to check your platelet levels with your doctor.

    Hello and thank you very much for the useful video. I am worried for my mother. She wasn't allowed to have the second dose of moderna due to number 40.000 of platelet. She then consulted a hematologist and she started medication to raise them. Today she is taking the vaccine with around 100.000 platelets due to the medication. I am worried both about the effects of the vaccine but also will this medication that she still takes in lower dose interfere with the vaccination in any way? Could you tell me your view on that? Because our doctor was a bit dismissive to explain this.

      Thank you for the kind words, and I am sorry to hear your mother has low platelets. I am not sure what medication was prescribed to your mother. I can look into it if you let me know. If the drug is boosting the megakaryocytes then it should not interfere with her immune response to the second vaccine. If the drug is boosting her platelets by suppressing the immune system your mother may make fewer antibodies than someone not on the medication but she could still make antibodies.

      Thank you very much. The medication is called medrol (16mg), she has taken 2 pills per day for 7 days and 1 per day at the moment. I was wondering would it be possible to check the level of response with an antibody test later on?

      @@stellaforhandle Medrol does suppress the immune system, but one pill a day will probably not affect her immune response and antibody count too much. Many doctors agree that if you have a chronic inflammatory condition like arthritis or low platelets to continue taking the medication prescribed to you. The experts at Cleveland Clinic agree this is the best choice. You could ask for an antibody count to check her levels.     My platelets are 135k-150k from last several years for unknown reason, I dont know if I have any clotting issue or not, shall I avoid vaccine at all? Or avoid only viral vector based vaccines (AZ, sputnik, jj, cansino)? Can I take the chinese Sinopharm or sinovac? Many thanks

      Your platelets are within normal range (150-400,000), so perhaps you have mild thrombocytopenia. It is a very personal decision to get the vaccine, and I suggest you consult with your primary doctor. If I were you, I would think about how dangerous it would be to contract COVID and compare it to the dangers of thromobocytopenia. For example, does your job expose you to a lot of people. Can you isolate for another 6-12 months? Do you have underlying conditions like asthma? If you do not have underlying conditions or a dangerous job perhaps it is best to wait. However, if you do have some conditions, jobs or at risk family members perhaps it is best to vaccinate. Several people who replied to the video said they are getting the Pfizer vaccine and plan to get a blood draw 7-10 days after their first shot to check platelet levels which is a good plan.     I am very scared of getting the vaccine. What will happen if my platelets drop so low again. My Hematologist told me years ago, I had no other choice, but to have a Splenectomy. I have been healthy for so many years. I’m more scared of ITP than Covid. Do you think the splenectomy will keep me safe from a possible relapse? Should I risk getting the vaccine? I don’t want to over work my lymph nodes, because sometimes they get so swollen and painful. I trust you , because you are a immunologist, and know so much about this nasty rare blood disorder.

      Thank you for sharing that with me Nora. I'm sorry you have to deal with that. It looks like a splenectomy for ITP is about 50-60% effective. That's honestly a tough call. You may be one of the rare people who should not get the vaccine. How long have your platelets been healthy levels for?     My blood platelets dropped after the first dose of the vaccine and I have never had that happen so my question is it was not very very low but will my platelets go up on their own

      Yes they will. Your megakaryocytes are in your bone marrow renewing your platelets all the time. Your platelet levels should be back to normal in 1-2 months. I would check in with your doctor and get another platelet count before your second vaccine.

    My son has just been told he has this. I immediately thought of the covid jabs he’d had. That’s how I ended up here. Although the hints at this are found in the comments, I cannot help but wonder if there has been an increase in ITP since. Someone, somewhere is counting.

    Last March after my first dosis of Pfizer vaccine I got bruises on arms and legs…on that time any doctor answers why this happened, I was scared, the bruises gone without any medication but now I’m scared to have the second one, also my two daughters got them with the same side effects. How can I make an appointment with you 🙏🏽 I need answers to prevent this please.

      Oh you should have your platelets checked by your regular primary care doctor. I would do that before your second shot. I'm not an M.D., I'm a PhD, so I can't order blood work.

    I had subdural hematoma in the past. I was taking blood thinner at that time. My doctor asked me to stop taking it. Then, bleeding was gone. Now, I can't decide whether I should take a covid vaccine or not. I have had hypertension, and taken telmisartan, ARB and amlodipine. I wonder whether I can take a covid vaccine.

      Sounds very complicated. I would recommend you ask your primary care physician to check your platelet levels before taking the vaccine. If they are above 150K then you would be eligible to take the vaccine. Several people in the comments here have taken the Pfizer vaccine without dropping their platelet counts. Have you had adverse reactions to any other vaccines?

It was ok at that time. I heard that Novavax vaccines will get FDA emergency approvals in September. What do you think of Novavax vaccine? I heard it was made of traditional method.       @@NK-gb3bk Novavax looks like a solid vaccine choice to me, especially if you haven't had an issue with the flu vaccine. You are correct that it is a close to a traditional vaccine because it is just a protein (the spike protein) from the COVID virus. So no mRNA(Pfizer and Moderna) and no viral vectors (AstraZeneca and J&J). It is grown in moth cells, but unless you are allergic to moths I don't see any issues. I am mostly kidding about the allergy to moths. Some people cannot handle the flu vaccine because they are allergic to chicken and the virus is often grown in chicken eggs. I wish you the best!       @@NK-gb3bk I also have hypertension and am on an anticoagulant. I am in the Novavax clinical trial. But definitely check with your primary healthcare provider before getting it yourself, we have very different histories. And never get your medical advice from some random dude in CZcams comments. I can say from personal experience that the vaccine reaction was about the same as I usually get from the flu vaccine, very mild. Two jabs are given 2 weeks apart.
      Novavax has a lot of advantages for use in places with less well developed medical infrastructure. Proven technology that the company has a couple of decades of experience with. Much easier storage and administration protocols for your providers.

    I have been getting Romiplostim injections almost every week. Juggling as low as 21L(21,000) to 164L for the past 4 years. I. E. One weekly blood test result was 114L and the following week test result turned 32L😰☹️ kept resulting this way😞. Strange that I have seen only 2 or 3 bruises on my body for the past 4 years.
    Also, took 80mg/day steroid for about 3 months when my platelets were down to 30Ls range. Then my face turned=huge pumpkin 🎃 + steroid did not work well in my body. The above injection=N Plate works better for me but the cost is just UN Believable. Plus, side effects in digestion and insomnia is just too much… Weekly blood tests + hard tissues on my arms (due to that hard to draw blood) gives me anxiety😰😰

    Thanks for answering some of my questions with this video. I want to blame the covid vaccine but don't believe that is fair.
    My very healthy son got his covid vaccine. About a month later he went in for is checkup. His blood draw had him at a little over 350k platelet count. He got his flu vaccine then about a month after covid Vax. His counts plummeted to 4k about 4-5 days after flu Vax. He rebounded to 160k after 2 months on his own. Then he plummeted again. They tried a heavy dose of prednisone and he went up and dropped again to about 19k a week after the prednisone. Now they are starting altrombopeg.
    Any idea to explain the cause and reaction of what might have happened?

      From my research, I would not recommend using aspirin if you have thrombotic thrombocytopenia. It can lead to excessive bleeding. Always consult with you doctor before changing your medication.

      That's a tough question. I think it depends on several factors and it is ultimately a personal decision. 1) If low platelets are not linked to an autoimmune disorder or if the low platelets are only mildly low (140-100K) then perhaps it would be appropriate to take the vaccine. Several people who watched this video did take the vaccine and planned a blood draw with their doctors to check their platelet levels 7-14 days after the vaccine. You could take this approach. 2) If you are prone to clotting the viral based vaccines like Johnson&Johnson and AstraZeneca may not be right for you. Although it is unclear if the people taking these vaccines are having more clots than the average population. 3) How dangerous is COVID to you personally? Do you work at home? Then maybe you can isolate yourself for another year and skip the vaccine. Do you have lots of family to care for? Do you have any underlying conditions that may make COVID worse (diabetes, smoking, ect.)? After you consider these factors you can make a plan with your doctor. Best wishes for your health.

      I would ask your husband's primary doctor. 190K is a normal platelet count. If he is on blood thinner, you may need to ask your doctor if mRNA vaccines (Pfizer and Moderna) are safer than virus based vaccines (AstraZeneca or Johnson and Johnson).       @@skandinikumar2770 Most people see symptoms of low platelets (bruising, bleeding, lethargy) 72 hours after the vaccine. If your husband experiences any of these symptoms or severe headache I would see a doctor immediately. If your husband does not see any of these symptoms, it would be safe to check his platelets about 10-14 days afterwards. Make sure they are checked before his second Pfizer vaccine! Best wishes

      Hi Akhil! Sorry your platelet count is on the low side. It's a tough call about getting the vaccine because if your platelets are low due to your immune system then the vaccine may lower your platelet count further. I would ask your doctor what they think, and then you could make a plan. I would say think about how risky your job is, perhaps wait and see if we can reach heard immunity. Maybe getting a single vaccine shot like the Johnson and Johnson is better for you, as long as you don't have any history of blood clots. Sit down with your doctor and make a plan.       @@pracharprasardotcom3770 I understand now. Giant platelets are different from megakaryoctes. Megakaryoctes live in your bone marrow, not your blood. If you have giant platelets in your CBC (which measures blood) it can be a problem because giant platelets don't work as well at stopping bleeding. The smaller, regular sized platelets work better to stop bleeding. Has your doctor give you any information or a diagnosis? Does anyone else in your family have giant platelets?

    I believe you said 150 million but that should be 150 thousand. In the US the range is 150-400 thousand.
    And I have ITP from the Hepatitis B vaccine which is not genetic.

    I had the first dose of Pfizer my platelets were 140 before and about 6 days later were 73 I was hesitant to take the vaccine at all and was concerned so was checking the levels

      I would check in with your doctor, and express your concern about the reduced platelet levels. Did your doctor check your platelets? Do you have a history of any issues with low platelets? Maybe you should wait before taking your second shot.

      @@Dramatohbandtahai MPV can improve in some people after the effects of the MRNA vaccine reduce over time By design the vaccine raises sub class igg4 levels and can pre dispose some people to auto immune illness If you are allergic to the vaccine you will be far more susceptible to long term blood related & auto immune concerns

People with pre existing autoimmune conditions should not have been vaccinated with the covid MRNA.
