Edit: Okay, I figured it out, he used his reverse Wheel of Fortune? (which does a random dice room effect) and I guess the dice room was One, which rerolls all your currently held items, and I guess one of his items became Missing No which eventually rerolled one of his items into TMTrainer and after it rerolled again, since TMTrainer cannot be rerolled, all of his item became "corrupted" which somehow killed him. What a sequence
