    I find that most of the time, when I have a hard time w/ IANB, asking the pt to open and close their mouth helps. This bc I noticed too that difficult IANB at the beginning of the procedure were completely numb by the time I finished. I find that it has more to do with them keeping their mouth open. Also waiting, but more bc of keeping mouth open

    yes regarding delayed onset i agree , smokers and alcoholics usually take longer time for anesthesia onset

      Do crown preparation under local anaesthesia but be careful.
      Give temporary crown always in such cases🙏🙏

    Thanks for this nice video sir. Can we buffer the LA by sod. bicarbonate in these cases or by use of bupivacaine can we achieve desired anaesthesia?

      Hi
      Bupivacaine is long lasting. But in hot tooth, we need an early onset rather than long duration.
      No idea about effect of buffering the LA.

      Usually NaHCO3 (being alkaline,pH=8.5) is added to buffer with abscess(being acidic,ph>=6)
      This property of sod.bicarbonate can be barely effective in hot tooth cases.

    I think we are mixing and miss understanding this symptom, most of the time it's the inferior molar, why it's almost not found in the other teeth, that because the anesthesia is not adequate

    Sir my pulp has died due to the use of depulp but the pain is still there...what should I do sir? Dentist is saying there isn't a missing canal in my molar as she found 3 canals...And I don't even feel pain when she insert every length of k files in my canal...what should I do sir?

      Yes and for that you have to reach pulp otherwise D pulp will not work🙏🙏

    I am little confused ..this case come under syptomatic irreversible pulpitis or necrosis ...Read somewhere that pulp necrosis also produce heat gases..Do both hot tooth and pulp necrosis both have hot sensitivity or produce heat gases or only hot tooth has hot sensitivity ??Thanx

      Hot pulp is mostly highly inflamed vital tissue

    Sir my teeth is paining too much on eating hot stuffs i have undergone RCT treatment on same teeth 3-4 month ago and I guess at that time I have hot tooth as when my dentist given anesthesia it was still paining after 2 blocks and he started treatment while I am in pain i have suffered a lot .
    So please tell me what should I do now is there any treatment for hot sensitivity should I go for retreatment or extraction

      Painkillers are quite effective in pulpitis. Premedication is always an aid to local anesthesia 🙏

      A little pain is expected for 2-3 days after RCT sometimes.
      If pain is severe or persists for long time, then we need to check xrays

      Patient giving symptoms of hot sensitivity relieved by Ice pack is indication of hot tooth.
      Only issue with pulp, rest will get anaesthetized.
      So, no issues with extraction 🙏🙏
