- An updatey type one. I've recently started a new job in acute medicine, my final rotation of FY1! I also finally succumbed to COVID-19 and have been isolating, and the results of the BMA UK Council Election have come out.

It's very refreshing to hear your push for pay restoration, industrial action and not worrying about public opinion. Can't wait to be a fellow junior doctor starting this August and join the fight!
Similar process in the US. Typically we have 2 acute teams that admit overnight.. Internal Medicine Inpatient Team and Family Medicine Inpatient Team. Only difference is FMIT can admit to telemetry/ regular wards but not ICU. Internal Medicine team can admit to any ward including ICU. Overnight sign out is at 5 am. Includes Intern (PGY 1) signing out to the resident (PGY2) who then modifies the plan and signs out to incoming chief resident (pgy3) the team does pre rounds around 6 am. Then around 8-9 am the team does full rounds with the attending/consultant who then will make final adjustments to the plan… then the rest of the day is about implementing the plan and updating discharge summaries and discharging folks before the next wave of admissions come in from the ER later in the afternoon… then you do it all over again the next day

I have/had an AMU rotation that I was looking forward to but I fell (lol) pregnant and I’m on maternity leave, so I don’t know what my rotations will be when I get back.

I got reffered to the gmc by my clinical supervisor as an FY1 now years later I'm a GP. It's a stressful job.
Hi Doc, what tips do you have for new doctors who might not know all the right medicines to prescribe at the beginning of their doctor career?
I'm right at the beginning of my career and would always say use the BNF (British National Formulary) or your local equivalent! For antibiotics most NHS trusts across the UK have slightly different internal guidelines anyway so it's always worth checking your own hospital protocols rather than trying to remember and getting it wrong.

For complaints the pathway to follow is Patient Advice and Liaison Service (PALS) for your local hospital or centre - they will be able to advise and help you process whatever the problem is

the plastic surgery consultant specialist in hand surgery at the r.v.i saved my life a number of years ago after my gp directly referred me to the plastic surgery department there directly. If you get the chance to work with them as an f2 the plastic surgery team at the RVI are incredibly competent and have a great background in emergency plastic surgery in Newcastle it would be a great rotation for you as an f2. As a former patient the team are top notch but the food is terrible be kind and offer to get the patients some proper food from some the franchised restaurants in the hospital

Clinical pharmacology drives doctors crazy. It basically involves a pharmacist rounding with you on patients that are on multiple medications and looking for drug drug interactions and making recommendations on what medications you should remove or add based on their knowledge of pharmacology.. in some places the clinical pharmacologist can even “change or modify” the doctors script though usually they just make suggestions… i thought it was funny how a pharmacologist who had never written a script in their lives was lecturing me about cytochrome p 450 interactions telling me to switch someone from tramadol to toradol for pain control because the patient could develop serotonin syndrome because he was also on an SSRI… my reply umm.. he’s got 1 kidney so no I’m not going to put him on toradol.. but thanks for having a crack at that dr stuff please stay in your lane thanks..
