      Hello-
      In the new year I'll be releasing videos on GB and the bile ducts and a renal ultrasound video (stay tuned).
      Most important factor when imaging the Gallbladder is to image in in 2 patient positions in a long plane, paying special attenntion to the neck of the gallbladder (where stones like to hide). The gallbladder wall should be measuring near the fundus (top of the gallbladder). The common hepatic duct (inside the liver) should be elongated as much as possible on top of the portal vein and color Doppler should be used to differentiate the duct from the portal vein. The duct should be measured in the A-P dimension (height) inner wall to inner wall. The common bile duct (outside the liver) should be elongated as much as possible in the sagittal plane, and/or evaluated at the pancreas head (in a transverse plane). Use Color Doppler to distinguish it from a vessel. Measure the A-P dimension, inner wall to inner wall. The PV should be evaluated with Color Doppler. Use an intercostal, coronal plane to image the MPV as it enters the liver (this window will give you a near zero degree angle- which is great for Doppler). The flow should be into the liver (above the baseline). To image the bladder pre-void, measure the length (horizontal measurement) x height (vertical measurement) x width (horizontal measurement) of the bladder (use Sagittal and transverse) and multiply that number by 0.523 to obtain a pre-void volume. Use Color Doppler to watch for ureteral jets (while the bladder is still full). After visualizing both ureteral jets and measuring the pre-void volume, then have the patient empty their bladder. When the bladder is empty, measure the length x height x width x 0.523 of the bladder in sagittal and transverse to obtain the post-void volume. Hope this helps!
