Prostatitis-Inflammation of Prostate, Type- Acute, Chronic
Cause- Infectious or Non-infectious
Infection- Hematogenous spread from UTI
Causative organism- E. Coli, Staphylococcus etc.
Structural & Functional abnormalities- Bladder outlet obstruction
Unprotected sexual intercourse- Rectal intercourse
Indwelling catheter
Dehydration
Immunosuppressed
Stress
Clinical Features-
Acute Prostatitis- few days to week
High grade fever with chills
Painful micturition, urgency, frequency
Retention of urine
Common in young age
Perineal heaviness with rectal irritation & urethral discharge
Sample of initial urine contain thread- turbid urine
On rectal exam-Tender & boggy prostate
Investigation- CBC, Urine culture, Ultrasound abdomen
Treatment-
Prolonged Antibiotics- 2 month
Avoid alcohol, sexual intercourse- 6 week
Complication-
Formation of single & multiple abscess- common in diabetic
Chronic prostatitis
Retention of urine
Chronic Prostatitis- 3 month or more
Mainly- Old age
Always associated posterior urethritis
Epididymitis
Perineal heaviness & discomfort
Pain in perineum, rectum, low back pain, leg pain.
Infertility (Sexual dysfunction )
On rectal exam-Tender & boggy prostate
After prostatic massage- voided urine shows prostatic fluid (15 or more pus cells)
Investigation-
Blood test- for infection
Urine test- Urine culture & sensitivities
Semen test- for infection
3 glass urine test
Other-
PSA(prostate-specific antigen)
Cystoscopy
Trans rectal ultrasound
Treatment-
Antibiotics= 2-3 week
Anti- inflammatory
Anti pyretic
Muscle relaxant
Alpha blockers
IV fluid- if dehydration

Mujhe prostitis ki problem hai penis base and testicles ke back side mein pressure feel hota hai micturition ke time pe .lekin 2 months se mujhe erection ki problem aa rahi hai aur ejaculation ke time pe semen ka volume km ho gya hai to kya ma'am ye prostitis ki wajah se hi hai.
MAMfor Bacterial prostatitis Dr. Recommend me azithromycin 1000mg once in a week for 5 months and levofloxacin 500mg for 10 days and then 250 mg for 20 days, but using double antibiotics I feel weakness in my body, is it ok. I have back pain issue also
