The performance I watched before this was Elliott Smith performing 'Waltz #2' where he gets to the "I'm tired" bit of the song and can't continue. I can imagine this song could be difficult to do if Tom is in a down period and finds it hard to voice the "right now, I'm happy to be alive" part. I'd probably lyric change it to "sometimes, I'm happy to be alive" on the days I was struggling.
