You can try to put rubber between the cover and the base so it doesn't rattle, maybe a bearing change can help or a new belt

Nice fix but isn't that a bearing below where you filed a groove and couldn't the filings enter causing further damage?

Вообще, лучше делать не 4 отверстия, а 3 с шагом 120 градусов. Будет стабильней.

You gotta be kidding!! By the looks of the fresh paint, that pulley is not very old. Take it back to where you bought it from and get a replacement!!
