Multi-Disciplinary Team meetings known as MDTs put our service users at the heart of all services provided in the community and are at the centre of the Sirona Care Model for how we deliver services. They bring together health and social care professionals working in primary care, community, and mental health care settings with colleagues from specialist providers such as end of life care to discuss the best way to support those with complex health and social care needs or who are vulnerable.
