      Hi, I've overbite and crowding in my lower jaw can it be corrected without extraction and will marpe help to get desired result? Does marpe will work in my case? I'm 20F. Pls let me know I'll be grateful to you!

    thanks for sharing the detail - ive been offered SARPE on the NHS but I'd rather try MARPE first (im a 33 year old male) just to avoid surgery - im happy if it doesn't work because I can just try SARPE after. Not sure NHS will do it though so may need to go external for MARPE.

    So I got this done with the MSE II. My surgeon told me to turn once a day. After a month my orthodontist said I can stop turning. I didn’t get a gap and my molars are extruded. Does that mean it didn’t work? Great presentation also.

    This is a helpful video. What is the earliest age you would do MSE vs RPE with a hyrax? I have a 12.5 year old daughter and trying to decide the best option. She does suffer with nasal congestion, that was the reason the MSE peeked my interest, but can you see nasal cavity widening with a hyrax too?

      I would use MARPE for maximum skeletal effect

      I have read all the studies and seen it with my own eyes and only the MSE expander offers skeletal expansion. RPE offers expansion but it is a V- shaped expansion where is more anteriorly than posteriorly

    Very well done and helpful. Thanks! I'm interested in helping a 7 year old with palatal expansion and pull the maxilla forward using these miniscrews. What length/size miniscrews would you recommend for a 7 year old? Would 10mm be enough/too much (in general)?

      I wouldn't use a MARPE on q child that small because of the thin palate and poor quality bone. A hyrax with bands or cemented acrylic caps would do

      @@alhuwaizi Any ideas for reverse pull headgear for 7 year old? I'd like to minimize dental effects and just pull maxilla forward. I thought a TAD would help but I guess not.

    What if you first open the palatine suture surgically and soon after apply the MSE?

      That would be SARPE - surgically assisted. In that case there is little benefit to use a MSE, you can use a regular Hyrax

      No, for that you need CCW of the lower third (upturns the nose tip)

      @@QuoteKingYt there was a study showing how force application would be predicted to change the maxilla. That showed greater change than 3mm however, which is what won moon said is the upper end of movement in adults. Also, idk if that study would translate into real life, but I'm gonna try it

    If get another look at the 5 apical radiograpgh at the 23:40 min in the lecture there is a horizontal fructure in left incisors 21 between the icisal third and middle third
    how could you manged that

      It is better to leave the incisors a few months to close spontaneously first. Yes use the facemask

      No, the Hyrax can not open suture in adults. Use TAD supported expander

    Hi doc! I had a question. I recently got a marpe with an orthodontist and I expanded asymmetrically where one side moved so far out it fell and my breathing only improved on that side. My ortho wants me to expand again once he fixes my bite and closes my gap in a few months. Is that possible later down the line? Once we begin expanding again to get the under expanded side to move it will be 8 months since expansion . I now realize I got a triangular expansion with the marpe and should have got a mse is it possible to get a mse to fix this issue? What are your thoughts?

      It is possible to use MSE now. Repeating expansion is Ok.
      The problem is how are you going to make the unexpanded side more liable to move? I prefer to do some bone cuts (osteotomy) to weaken that site

      @@alhuwaizi Hello doc! Thank u for the reply! I know you said repeating expansion is ok, but is it ok that it’s been 8 months since initial expansion? And how does the missing bone fill in? Does a new suture form as the new bone builds between the two halves or is it just one solid bone with no suture in between ? Also where would the cuts be made to help the unexpanded side loosen? My ortho told me the side that over expanded has denser bone, could that explain anything? Lastly I’m concerned the over expanded side will move again once we start expansion. Is there a way to install the mse so that only one side expands. And can the mse be limited to just posterior expansion because my marpe already expanded the anterior portion of my upper jaw giving me a triangular expansion.

    I got my expander in 5 days ago. And after watching your video, I am wondering why my screws are left so long…The expander is tight to my palate but the screws are ~2mm too long it seems like
    Is there a reason for my orthodontist to have done that?

    Selam Dr. Akram. Do you know about an orthodontist that does the same in Europe? I a have been visiting several dentist and a few orthodontist, but nobody mention this method. Actually only a few mention skeletal expansion at all here. This have affected my life very negatively and I have not been able work for 2 years now.
    How much will this cost, with braces afterwards or without braces? Thank you for sharing this information and for your honesty. It seems like you are genuinely interested in your profession and dare to try out new technologies.

    The gap created post treatment will it go permanently using clear alinger?

      It will close spontaneously as the transseptal gingival fibers pull the centrals together but a fixed appliance is needed to upright them

      @@joejacob9299 it can be closed with clear aligners, but difficult. There is always a chance of relapse so we must overexpand and wear a retainer

      Two or three a day, but I prefer a buccal corticotomy to weeken the resistance

      It becomes loose and falls. Sometimes, it can be swallowed. It will come out. No worries.
