I have given thrombolytics for suspected PE once in 25yrs in ER. Recent knee surgery 35yr old female. Presentation increasing SOB over that morning. She was ER approx 45minutes before coding. Pushed TPA,...... couple days later she was up walking. 6months later I saw her in ER for leg swelling.
Pushed? for APE it's to be given over 2 hours. Pushing it would be off label use and incur much liability.
Did a CT scan confirm it was a PE?
@@boz287 I had a physician push tpa in a patient with a suspected PE that coded 15 minutes after getting to the room

Awesome videos, Mark! Thanks very much for these. I take my exam on Monday and have been using these for the past week as some final prep/review before the test.
In regards to pushing tPA on a coding patient: I've seen it done twice in three years, and both times we pushed tPA d/t finding a clot in transit in the RV via bedside ultrasound. We pushed tPA and voila, PEA became ROSC within a minute. Pretty amazing stuff. Granted, we have some pretty skilled ultrasound clinicians in our (large, university) ED who have typically done an ultrasound fellowship or have been teaching ultrasound fellowships for some time. Not everyone has this resource, but cool nonetheless!

I have seen doctors identify a saddle PE/heart strain on a bedside US in the ER who decompensated and coded get TNK! Ive seen it done several times
