1. He had a totem.
2. When his totem popped, he was already holding gapples (and spamming them) so there would be no difference.
3. He wouldn't have time to equip his shield because 1) It wasn't in his hotbar 2) His totem popped so he was extremely low and the only things that kept him alive for a few more seconds were the gapples. If he wasted just a bit of time to get that shield in the offhand, he would have died anyway.
