I really like how you explain the reasoning in a simple way that is easy to understand. Also not just explaining why the answer is correct but why the other answers are incorrect.

I have a literal library of book and pull something out randomly, I’m sorry but I could not even begin to tell you which I pulled from not to even mention my online catalogue. A great for you to start though would be “your best grade” as it goes by subject and not review mixed.
Regarding the question on when the nurse's mentor should step in......can someone advise me on why the "nurse leaving the patient to report to nursing station" is not a possible answer? I would assume that the mentor would step in because the patient interview is not complete at all...... thanks

You need to know when the pain started first before you ask them to describe the pain.
