O Mary, if death is so fearful even for the just, how much more so for a sinner like me, who has deserved hell many times over. Mother of Perpetual Help, you are my only hope. As the advocate of sinners and patron of the dying, I place my sinful soul in your blessed hands. Dispose of me as you see fit, but protect me from a sudden and unprepared death.
Reject all my other prayers, if you will, but grant me this one: a good and holy death. Let my final moments on earth be peaceful and may they be the fulfillment of all the prayers I offer to you until the end of my days. When my last hour approaches, O my good Mother, grant that I may receive the sacraments of the Holy Church with worthy disposition. Come to my aid, comfort me, and defend me from the attacks of the evil one.
May your name, together with that of Jesus, be my last thought and my final sigh, so that I may depart from this life and enter heaven to sing of your mercies. Come, O Lady of Perpetual Help, be with me at the hour of my death. Amen.
