The disconnect was caused by either using the space bar when swimming (not joking), or being slowed by the frost bolt. It can also happen when warriors use charge underwater. Basically abilities and effects that modify your movement speed when you're under water has a high chance to disconnect you, either immediately or a "soft dc" where chat still works but everything else just stops. It's super scary.
