The 780G is great. I love it. I reach 85-93% time in range. Love it.
Can you compare it to the 670g ?
Medtronic should put it's money where it's mouth is. Sorry for being a downer but as a parent of a 9 year old T1, I was woken up again in the middle of the night because of an unnecessary
pump alarm. He has the 670g with guardian 3 and this was the second time this week it gave an error in the middle of the night that shouldn't happen. These are alarms like "enter a BG to continue automodus" (why? the sensor is accurate and he's sleeping so the bg is pretty constant) or "The pump has been on max insuline for 3 hours" (Good, since it got it down from a high to now 8, so it's doing it's job!). The last one is then followed by an alarm that it exited automodus and an additional alarm to enter a BG again (again, why? the sensor is close enough). This happens on and off, I can pin point a 6 month window last year where I had night time alerts 2 of the 3 nights. Only 25 % per highs or lows and I think of those only 5% actually would need interaction If the pump was a bit more aggressive dealing with highs. We use the on board correction calculation and that works well, but why do we have to do it?
Yes, the algorithm works, my son has a TIR of 70+% with lower highs and not many terrible lows. The extended 7 day infusion set is great. But the sensor needs 4 calibrations in the first 16 hours (so guess what my night was like when I had a sensor failure at 22:00 hours) and takes 3,5 hours at a regular change to be effective (the transmitter needs 1 hour charge time and automodus can only start after a 2 hour warmup and 30 min check time). The amount of fingerpricks is very close to a pen and BG meter user this way...
So my trust in medtronic is not high at the moment. We're still a year away to the end of the contract, but my doctor can't help (since it does work) and medtronic is staying silent on the whole night time issues thing. Many have walked away from medtronic because of the 670g alarms. I heard a rumor through our diabetic nurse that they might like to offer the 780g when there is still 6 months left, if we take it for the next 4 years.
The 780G might be great and the guardian 4 a big improvement (only one calibration instead of at least one every 12 hours (and I won't mention the number of BG's it asks for). But how can I expect to take this risc if we're stuck with a new pump that ends up having the same problem?
If they truly believe in their product, offer it to all current 670g users as a direct replacement to the not so great 670g until the end of the 4 years. If this really works better, I won't hesitate to continue, but now I'm stuck between choosing 6 months of additional sleep deprivation or an unsure path with potentionally 4 years more broken nights. And the ypsopump with camaps and dexcom combo looks very nicely, way more advanced in appearance compared to the ipod knockoff look of the 780g.

My point is that the figures he talks about are mainly people that aren't very stabile on MDI have great benefits with 780g (which is a easy group to improve upon, and all other manufacturers do the same comparison), but that it is unclear what the added value is compared to 670g users and that they are doing nothing to help them with a better system (with less alarms and calibration needs).

It's amazing. I can imagine some improvements, especially the ability to use the prolonged bolus in smartguard but generally it works nicely for me, especially with Lyumjev. It was much worse with NovoRapid - the absorption was too slow with it. Now I'm somewhere about 90% TIR.
What would make a great improvement for me now, I believe, and it's for all pumps - is a switch to U200 insulin. With my poor sensitivity and many years of using pumps, it's taken a big toll on my subcutaneous tissue. I can imagine cutting the volume of the insulin to half would greatly improve the outlook. The pumps are precise enough for that.
I’m in Chicago, my emotions (PTSD) have always had a big impact on my glucose levels yet too many doctors they point their finger at me saying it’s my fault that I’m doing something wrong my only regret that day is that I didn’t go off and tell her what she can go do with herself, and the worst part it was at a big hospital one that’s supposedly known for good diabetes care I will never go back to Rush again not after how I was treated. I’ve never heard any Dr say it’s not our fault so now I’m going to try and stop crying
When will you figure out how to work the US FDA? I have used your pumps for 30+ years and this has been a huge f***up!
Hi I've gotten my 780G for a week now, I'm completely out of control even though my daily carb intake is absolutely minimal (around 40-50g a day total carb intake) I eat carbs like brown bread, white rice, buckwheat nothing too extreme and I avoid fast food and sweets. My TIR is around 50%, and smartguard doesn't make correct adjustments. My settings at the moment as calculated by my health provider are: carb ratio 1:12, insulin sensitivity: 49 active insulin time: 3:30h, when I was on the pen my carb ratio was 1:15 and sensitivity closer to 70 and insulin time around 5h, if I put these numbers into my pump my BL gets even worse. Even though I see the potential of this pump I'm not fully conviced at this moment that this a good choice for me.
I'd like to see medical patents become dis allowed. Only then can the ultimate tech be made & released. We're still in a world of ego monetary competition
I have been a type 1 diabetic for 50 years. The 670G and 770G are Miraculous compared to what I used in 1972!
The 780G has proven itself in Europe for several years now. What's the problem with the FDA? The 780G has proven to be safe and highly effective in keeping T1Ds closer to normal blood sugar levels. We are tired of waiting, at least I am! When I upgraded from the 670G to the 770G, I was told that in a few m onths the software would be upgraded to the 780G. Ever so often the time range is extended by a few months. Now it's been over two years! It's not that I'm not grateful for what I have, it's knowing there's something even better that we have been promised and have waited for a long time when it's been used and succesfully proven in other countries.
I have had the 780g for over a year and am absolutely grateful for the pump but I am frustrated that the nhs won’t give me the cgm to go with it I don’t understand why they give us only 50% of the equipment
That is not good. There is now national CGM funding- not sure which UK nation you are in but there has been a lot of progress over the last year. I would recommend speaking to your consultant to see if you can get CGM reimbursed.

Great info. Although Medtronic have always had a fantastic pump, I’m like others complaining. CGM was only introduced in Australia last year yet we’ve had the 780 for quite some time. Desperately waiting on guardian 4 as I get woken most nights for ridiculous calibrations. Previous to CGM my control was very good, so I couldn’t gain lots as has been mentioned above. I spend most time above 85% TIR now. But disappointed in Medtronic’s sales pitch saying on the 780 closed loop system you will make 180 less decisions? I spend more time deciding what I’m going to enter now than I ever did before CGM!

Sometimes you want to put a mark of the carbohydrates you eaten (for example, 5 grams) like xDrip can, so that it is shown on the graph and you know what and when you ate. It would also be interesting if the pump displayed active carbohydrates too and took them into algorithm. The algorithm considers only your insulin and how much you ate for the planned meal. But how much you ate at low he does not count and does not keep records. It also considers a injection, then you ate something but you didn’t eat anything, you just inject because of high sugar. It is not correct. Or maybe just add short notes to the graph. Sometimes it could be helpful. And it will be nice to see collaboration with libre 3 :D
I have the 670G with guardian 3 and it did change my life. Let's see how long it takes to get this new pump and cgm to Mexico. Can't wait to test it

Keep up the good work 👍only had the kit for a couple of weeks and it's very clever. Shame i have a chromebook and a non mainstream android 11 phone as the app and the software doesn't work 😕
Software updates are via the app not laptop. I totally agree, number of supported phones is very very low.
I'd like to know more about this new sensor? It looks self contained. How long will it last? Most importantly when will we see it in the U.S.?

bro 1.4% is really accurate estimate. Went from around 8.4% to 6.9% when I switched to 780g with guardian 4.

we got allergic to the Guardian 4 and it started showing incorrect readings, so we went back to the Guardian 3.

@@nejc509 I think it's because of the material of the sensor's antennae. When we had libra 2. A year later we had something similar.
What do you do if you want to swim? My experience has been an entire failure trying to use the 780G sensors recommended by Medtronic. This whole idea is a failure. I want a sensor much like the Dexcom sensor. 1 fully intact waterproof unit that allows a person to swim and immediately move out of the water, gather information and bolts .
Mr Cohen I appreciate what your saying however it would have been great if your company had trained its people like customer service reps to tell their patients for the past year instead of putting their guardian sensors 3 on their abdomen and only get 4 days out of them tell them to put them on their upper arms! I’ve been thru hell and back with the past year calling Medtronic several times and nobody advised me of this. Just found out about it today! Interesting the guardian 4 sensors must only be put on one’s arm! Wake up already!!
i had the freestyle libre 2 and had a very negative experience with it. i bought two sensors and after a few hours they stopped working. had to pay them anyway. not nice.
i am looking for an alternative. relative accuracy should be good, absolute accuracy is not that important.
no insulin injections needed, just need to lower my blood sugar AND insulin for weight loss.
should guide my experiments with fiber to grow more l cells
If you call customer support they will replace them.
Medtronic Guardian has the worst accuracy
Thankfully I have learned to not rely on the Medtronic sensor; after years of damage.
Very frustrating when the sensor reading is 140, and the actual finger stick reading is 280.
Or the very common reading of below 40, when the actual blood reading is 90.
It's a miracle I'm still alive after the inaccurate sensor readings from medtronic.
Dexcom G4 from 12 years ago was much more accurate than the Medtronic sensor that is available today

670g algorithm is/was NOT good. Continued micro bolus delivery when pump sees 40mg/dL is not my idea of GOOD.
This is all great and wonderful. But here in The Netherlands, this tech is inaccessible for the vast majority (maybe 80 percent or more) of people with T1D. Why? Because these fossil jerks in the "Dutch health institute" (ZorgInstituut Nederland) and the Minister still don't see any scientific evidence to fund Continuous Glucose Monitoring for improving quality of life.
Only people under 18, wanting to get pregnant or who have their life already ruined with complications can get it.
The vast majority omly gets FGM (which is sub-ootimally useable in DIY looping setups).
Oh and while Medtronic is at it, enlightening the 80's-knowledgeable policy makers in the Netherlands, can they please open up the system to third party integrations like DIY looping?
