A standardized clinical examination of the lumbar spine is critical in the evaluation of patients with low back pain. Progressing in an orderly fashion through the different components of the examination ensures accuracy and repeatability.

As a physiatrist I would initially examine the patient in a standing position. The reason for this is that you can see if there’s a straightening of the Lordotic curve. Muscle spasms of the lumbar paraspinal muscles could cause a straightening of the lordotic curve as well as scoliosis. I would also have the patient to perform flexion and extension in a standing position to evaluate for discomfort or pain. Following this I would then examine the patient in a supine position on the examining table.
