It isn't necessarily due to cosmetic surgery. Her changing looks are mainly if not solely resultant from lupus and the associated medication. This life long lupus management medication can result in a 'moon face' amoung many other side effects. She's still gorgeous obviously and I feel for her having to experience all the changes that lupus continues to cause.
